Habitual dislocation of the patetla medialiy (A rare complicotion of the Hauser operation)

Abstract

Habitual dislocotion of the patella is characterized by lateral displacement of the patella as the knee is flexed and returning to its original site as the knee is extended. So far in literature no case of habitual patella dislocation has been reported in which patella is medialiy displaced when the knee is flexed. Several cases of the medial subluxation of the patella have been reported which have resulted from the complication of the surgical treatment of this disease, yet as it is the case in the habitual dislocation of the patella, no case has been observed in literature in which patella has completely been dislocatd medialiy as the knee is flexed. We have therefore considered to be interesting to observe a habitual dislocation of the patelta medialiy we have seen in on of our patients on whom Hauser operation was performed for therapeutic aims, in consequence of which we have presented in detail the reasons why a medial dislocotion has occured and the applied surgical technique for the correction of this defect. Taking opportunity with this case we have discussed the principles to be complied with in case of habitual dislocations of the patella and various complications that might arise in case Hauser operation is adapted for therapeutical purposes and have indicated that Hauser operation has no place and even contraindicated in the treatment of this disease.

PATELLA'NIN HABİTUEL MEDİALE DİSLOKASYONU (HAUSER AMELİYATININ NADİR BİR KOMPLİKASYONU)

Öz

Habituel patella çıkığı, diz fleksiyonu gelirken patellanın laterale doğru yer değiştirmesi, diz ekstansiyona gelirken ise yeniden eski yerine dönmesi ile karakterize olur. Bu güne kadar literatürde, diz fleksiyona gelirken patellanın mediale doğru yer değiştirdiği habituel patella çıkığı olgusu bildirilmemiştir. Bu hastalığın cerrahi tedavisinin komplikasyonu sonucu ortaya çıkan birkaç patellanın mediale sublukasyonu olgusu bildirilmiş, ancak tıpkı habituel patella çıkığındaki gibi, patellanın diz fleksiyona gelirken mediale tam disloke olduğu bir olgu literatürde bulunamamıştır. Tedavi amacıyla Hauser ameliyatı uygulanan bir olgumuzda gördüğümüz patellanın habituel mediale dislokasyonu bu nedenle ilgi çekici bulunmuş, mediale dislokasyonun neden ortaya çıktığı, bunun düzeltilmesi için uygulanan ameliyat tekniği ayrıntıları ile sunulmuştur. Bu olgu nedeniyle hatibuel patella çıkığı olgularında uyulması gereken tedavi ilkeleri, tedavi amacıyla Hauser ameliyatı uygulanması durumunda ortaya çıkabilecek çeşitli komplikasyonlara değinilmiş, bu hastalığın tedavisinde Hauser ameliyatının yeri olmadığı ve hatta kontrindike olduğu belirtilmiştir.
