MANAGEMENT OF THE STIFF KNEE AFTER TRAVMA By JÜDET TECHNIQUE

Abstract

In fractures of the shaft of the femur especially or the middle 1/3, fibrous tissue arising from the fracture site contracts and holds the quadriceps muscle. As a result knee stiffness occur in extension. In such patients changes take place at the same line in the recessus underneath the quadriseps muscle (synovial deadend) which secure the knee joint motion at ease. A long standing external fixation applied or the treatment of the femur shaft fractures causes the increase of these changes. The knee joint restriction in extension results with many functional disorders which should be treated. Among many kinds of operation techniques performed for the knee joint restriction the one I applied to six patient in our clinic and took very good results is JÜDET operation. The Operation Technique Is Likewise: a. An eight or twelve centimeters long incision anterolateral to patella is made to free the synovial deadend and the patella. B. In order ta slide the vastus lateralis musele from its origin and to detach the quadriseps muscle another long incision along the lateral aspect covering 2/3 of the thigh beginning from the greater trochanter si made. Physical treatment is begun on the first day after operation and weight bearing is allowed in 3 or 4 weeks.
