Abstract

In any operation on the first toe, the metatarsophalangeal joint, flexion should be unimpaired, extension of this toe is severely disabling. As a rule the methods in current use are more conservative than those formerly employed. In the presence of only moderate hallux valgus deformity, particulariy in young individuals we have employed McBride operation with good results. In our opinion at the procedure there is no need to remove the medical exostosis and isolation of the tendors of abductor hallucis and the medial part of flexor brevis from original insertion points of the first phalange, in order to resulting in a more medial position. The Keller operation is employed in practically all cases in which the excision of bone is required to shorten the orseus structures, relax the contracted soft tissues and place the joint in a satisfactory position, and especially in hallux rigidus cases. The Mayo procedure should be employed in cases where the first toe does not function, and by partial removal of the metatars head may not create any worst walking conditions in elders or flatfoots with generalized arthritis.
