The safety of pedicle screw fixation in the thoracic spine

Abstract

Objectives: Thoracic pedicle screw fixation has many advantages over other methods of spinal fixation in treating various conditions. We evaluated the safety and efficacy of pedicle screw fixation in the thoracic spine. Methods: We retrospectively reviewed 144 patients (65 males, 79 females; mean age 38 years; range 9 to 82 years) who underwent posterior thoracic pedicle screw instrumentation. A total of 827 thoracic pedicle screws were inserted to the thoracic spine (T1-12) by the same senior spine surgeon using the free-hand technique. Indications for thoracic spine surgery were deformities (n=47, 32.6%), metastatic or primary tumors (n=15, 10.4%), spinal infections (n=7, 4.9%), degenerative diseases (n=13, 9%), and spinal trauma (n=62, 43.1%). Screw containment was assessed by three independent reviewers on postoperative plain radiographs. In addition, thin-slice computed tomography scans were obtained in 21 suspected cases (29 screws). Results: Screw containment was found in 94.3% (780 screws). Incorrect screw placement was found in 47 screws (5.7%), including 31 screws (3.8%) demonstrated by plain radiographs, and 16 screws (1.9%) demonstrated by thin-slice computed tomography scans. More than half of the faulty screws (n=24, 51.1%) were detected in scoliosis patients, especially on the concave side of the curvature (83.3%). The most frequent site of faulty screw placement was the T8 level (21.3%). No symptoms or complications occurred related to faulty screw placement. Conclusion: The high accuracy of thoracic pedicle screw placement (94.3%) using the free-hand technique suggests that pedicle screw fixation of the thoracic spine is safe, reliable, and useful in the treatment of all types of spinal diseases.

Torakal omurgada pedikül vidası ile tespitin güvenilirliği

Abstract

Amaç: Çeşitli omurga sorunlarının tedavisinde torakal pedikül vidaları ile tespitin diğer tespit yöntemlerine göre birçok avantajı vardır. Bu çalışmada torakal omurgada pedikül vidası ile tespitin güvenilirliği değerlendirildi. Çalışma planı: Torakal omurgaya yönelik posterior girişim ve enstrümantasyon uygulanan 144 hasta (65 erkek, 79 kadın; ort. yaş 38; dağılım 9-82) geriye dönük olarak incelendi. Olguların torakal pediküllerine (T1-12) aynı cerrah tarafından serbest teknikle (free-hand) toplam 827 pedikül vidası yerleştirildi. Hastaların ameliyat nedenleri 47 olguda (%32.6) deformite, 15 olguda (%10.4) metastatik ve primer tümörler, yedi olguda (%4.9) omurga enfeksiyonu, 13 olguda (%9) dejeneratif hastalıklar ve 62 olguda (%43.1) omurga travması idi. Vidaların kemik içinde olup olmadığı düz radyografiler üzerinde üç gözlemci tarafından değerlendirildi. Gerek duyulan olgularda (21 olgu, 29 vida) ise ince kesitli bilgisayarlı tomografi kullanıldı. Sonuçlar: Pedikül içinde normal yerleşim gösteren vidaların oranı %94.3 (n=780) bulundu. Düz radyografilerde 31 vidanın (%3.8), ince kesitli bilgisayarlı tomografi incelemesinde 16 vidanın (%1.9) pedikül dışında olduğu belirlendi. Pedikül dışında bulunan toplam vida sayısı 47 (%5.7) idi. Hatalı yerleşim gösteren vidaların 24’ü (%51.1) skolyozlu olgularda, özellikle eğriliğin çukur tarafında (%83.3) görüldü. Hatalı yerleştirilen vidalara en sık (%21.3) T8 omurunda rastlandı. Olgularda torakal pedikül vidaları ile ilişkili herhangi bir semptom veya komplikasyon görülmedi. Çıkarımlar: Çalışmamızda serbest teknikle yerleştirilen torakal pedikül vidaları %94.3 isabetle kemik içine yerleştirilmiştir. Elde edilen sonuçlar her türlü omurga rahatsızlığının tedavisinde torakal pedikül vidaları ile tespitin emin, güvenilir ve yararlı bir yöntem olduğunu düşündürmektedir.
