Mechanical strength of antibiotic-loaded bone cement: A biomechanical study with Cemex bone cement, teicoplanin and tobramycin
Abstract
We used tobramycin due to its broad-spectrum, and teicoplanin due to its effectivity against resistant Gram-positive organisms to investigate the effect of antibiotics on the mechanical properties of bone cement. We added 0.5g, 1g, 2g, 3g and 4g of either teicoplanin (Targocid ¨, Hoechst) or tobramycin (nonsterile powder, Lilly) to Cemex bone cement and measured its compressive strength according to the standards set by the American Society for Testing and Materials (ASTM, F 451-95). For each antibiotic concentration in each group, cement was polymerized in special molds providing us with test specimens which were cylinders 12 mm high and 6 mm in diameter. One group contained no antibiotics and served as the control group. Compressive strengths were 98±7 MPa in the control group; 94±7 MPa for 0.5g, 92±7 MPa for 1g, 86±5 MPa for 2g, 78±8 MPa for 3g, 58±9 MPa for 4g in the teicoplanin group; 95±5 MPa for 0.5g; 88±8 MPa for 1g; 86±7 MPa for 2g; 85±9 MPa for 3g; 80±11 MPa for 4Êg in the tobramycin group. Compressive strength was above the minimum standard in all groups except teicoplanin with an antibiotic concentration of 4Êg/40g cement. 1, 2, 3 and 4g specimens also had a statistically significant weakening in mechanical strength when compared to the control group. When mixed according to the method described, addition of up to 2 grams of teicoplanin or tobramycin to Cemex bone cement neither weakens its compressive strength nor alters its handling characteristics substantially.
