Autografts in anterior cruciate ligament surgery
Abstract
Bone-patellar tendon-bone grafts (BPTB) and Hamstring tendons (HT) are the mostly used autografts on the anterior cruciate tendon surgery. There are some factors which must be considered carefully when these two grafts are compared. These factors are; the biological convenience of the graft used in the organism, the morbidity occurred on the donor side, initial graft strength, structural characteristics of graft, graft's initial fixation strength, duration of remodelation of graft in the knee, duration of healing of graft in the bone tunnel, graft length and rehabilitation programme of the postoperative period. It is difficult to choose the graft for surgeon since there are a lot of alterations. However, it must be taken into consideration that 4 times folded HT graft has much more initial strength and contains more collagen fibrils. Besides it has been demonstrated that BPTB grafts which are fixed with 9 mm Kurosaka screws have stronger initial fixation. Apart from this, the weakest points of both grafts' at early postoperative period are fixation points. After 8 weeks, both grafts become weaker than fixation points. Accelerated rehabilitation programme can be used for each graft at the postoperative period. Prospective clinical studies have demonstrated no significant difference for the reconstruction of anterior cruciate ligament for both grafts. Hence, HT graft taking is technically easier and can be applied in a short time.
