Divergence of interference screws in arthroscopic anterior cruciate ligament reconstruction
Abstract
Patellar bone-tendon-bone graft is presently the most commonly used graft for anterior cruciate ligament reconstruction. The bone plugs of this graft are often fixed in the osseous tunnels with interference screws. Between 1993 and 1996, in our clinic, we applied 98 arthroscopic (single-incision) ACL reconstruction using interference screw fixation. In this study, we reviewed the divergence of the bone plugs in femoral and tibial tunnels with interference screws retrospectively in lateral and anteroposterior plane. Femoral divergence occurred more frequently in the lateral plane (44.5%) than the anteroposterior plane (22.3%). There were no early graft failures noted clinicaly or KT-1000 arthrometer testing. The divergence of bone plug in tibial tunnel with screw was less when compaired with femoral divergence. If the divergence of femoral screws is higher, it leads to early clinical failure in arthroscopic ACL reconstruction. When screw is placed in a divergent fashion, stresses on the graft-screw-tunnel sonstruction will concentrate distally and result in a wedge effect and early graft failure. If screw is placed divergently and stability of fixation is provided intraoperatively, there is no need to change in early range of motion protocols and rehabilitation.
