Abstract

Rotational knee instability of 33 patients in Gazi University Medical Faculty Orthopaedics and Traumatology clinic, Ankara, Turkey with a coverage between March and August l987 is reported. We avaluated the subjective complaints by anterior and posterior drawer tests (AMRI-ALRI-PMRI-PLRI), jerk test and stress tests of abducton. 13 cases only arthroscopically and 2 cases as well arthroscopically as arthroscopically examined. The results of clinical and arthroscopic diagnosis have been compared. If a knee has a problem of rotational instability; whether the anterior cruciate ligaments is intaci or not, in a long period the risk of degenerative arthritis arises. If there is no rotational instability quadriceps will from pretection for the knee. As a results, even though the anterior cruciate ligament is intact, the rotational instability of the knee possibly could occur. Carreful evaluation and treatment of signle and multi-plane rotational instability may decrease the risk of degeranative arthirits.

Abstract

Mart-Ağustos 1987 süresi içinde Gazi Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalında görülen 33 diz bağ instabilite olgusu çalışmaya alınmışır. Hastaların subjektif şikayetlerini takiben tek ve çok planda rotasyonel instabiliteler, anterior ve posterior çekmece (AMRI-ALRI-PMRI-PLRI), jerk test, lachman, Ritchey, Abduksiyon, Adduksiyon stress testleri uygulanarak değerlendirilmiştir.Bunlardan iki olguya artroskopi ve artrografi, diğer 13 olguya yalnızca artroskopi yapılmıştır. Klinik muayene bulgusu ile diagnostik artroskopi sonuçları karşılaştırılmıştır. Rotasyonel instabilitesi olan bir dizde, anterior cruciate ligament yırtığı olsun yada olmasın ileride meydana gelecek dejeneratif artrit şansının yüksek olacağı, anterior cruciate ligamentin kopuk, rotasyonel instabilitelerin olmaması halinde ise kuvvetli guadricepsin her zaman dizi koruyabileceği unutulmamalıdır. Sonuç olarak; ön çapraz bağ sağlam olduğu halde rotasyonel instabilite meydana gelebilmektedir.
Tek-çok planda rotasyonel instabilitenin yeterli değerlendirilmesi ile ileride meydana gelecek erken dejeneratif artrit riski azalacaktır.
