Results of closed intramedullary nailing in the treatment of the tibial shaft fractures

Abstract

Between 1981 and 1988, 314 tibials shaft fractures were treated in Orthopaedic and Trumatology Department of Ege University, Medical Faculty. 46 of them underwent closed intramedullary nailing with reaming. Infection was observed in one patient with decubitis problem, when early operation was performed mandatory. The period of healing according to our evaluation in between the date of the operation and the time when the patients becomes ambulatory. The average time to heaIing was 20 weeks. Rotation and angulation deformity, malunion and nonunion were not observed in the 21 patients who were evaluated in for the last control. There is no universally accepted method of management of tibial shaft fractures. Closed intramedullary tibial nailing with reaming incorporated many advantages of nonoperative therapy with increased stability of intemal fıxation. This technique allows early wight bearing.

Tibia cisim kırıklarında kapalı Küntscher yöntemi ile tedavi sonuçlarımız

Abstract

E.Ü.Tıp Fakültesi Ortopedi ve Ttavmatoloji Ana Bilim Dalı'nda 1981 ile 1988 yılları arasında 314 tibia cisim kırığından 46'sında intramedüller kapalı Küntscher tekniği ile tedavi uygulanmıştır. Bası yaraları olan birinci derece açık kırıklı bir hastada erken yapılan girişimden sonra enfeksiyon görüldü. Değerlendirmemizdeki kırık iyileşmesi, ameliyat tarihi ile tam desteksiz yüklenme arasındaki süredir. Bu ortalama 20 hafta olarak bulunmuştur. Son kontrol çağrısına gelen 21 olguda klinik ve radyolojik değerlendirmede rotasyon kusuru ve kaynamama görülmedi. Tibia cisim kırıklarında, halen değişik tedavi metodları kullanılmaktadır. Uygun endikasyon konuları olgularda, kapalı intramedüller Küntscher tekniği ile tedavi seçkin bir yöntemdir.
