Abstract

We have revİewed ten cases of femoral neck pseudoarthroses treated with Pauwels Y osteotomy at the Department of Orthopedics and Traumatology of İstanbul University, İstanbul Faculty of Medicine, between 1979 and 1987. Union was achieved in all cases. Four of the hips had varying degrees of avascular necrosis before and after the operation. Avascular necrosis developed postoperatively in two of remaining six hips. Clinical results were excellent in one, very good in three, good in five and poor in one hip. We concluded that, Pauwels Y osteotomy could offer good results when the primary problem is non-union.

Abstract

1979-1987 yılları arasında İ.Ü.İ.T.F Ortopedi ve Travmatoloji A.B.D'da kollum femoris psödoartrozu nedeniyle tedavi gören 30 hastadan, yapılan mektupla çağrı üzerine kliniğimize müracaat eden ve geç takibi yapılan 1 0 hasta klinik çalışmamıza dahil edildi. Daha önce Pauwels Y osteotomisi uygulanan tüm kalçalarda tam kaynama elde edildi. 10 kalçanın dördünde ameliyattan önce ve sonra çeşitli derecelerde avasküler nekroz mevcuttu.
Pauwels Yosteotomisinden önce normal olan, kalan 6 kalçanın ikisinde post-op avasküler nekroz görüldü. Sonuçların klinik ve radyolojik değerlendirilmesinde Askin ve Bryan tarafından modifiye edilen kriterler kullanıldı. Buna göre bir olgu mükemmel, üç olgu çok iyi, beş olgu iyi ve bir olgu kötü olarak değerlendirildi. Kaynama yokluğu primer problem olarak kabul edildiğinde, Pauwels Y osteotomisinin uygun bir tedavi yöntemi olduğuna karar verildi.
