Induction of healing of large segmental defects of long bones in rabbits wiht transplantation of allogenic demineralized bone matrix (DBM) and autogenous bone marrow

Abstract

We studied the effects of allogenic demineralized bone matrix (DBM) and autogenous bone marrowon the repair of large femoral diaphyseal defects in a rabbit model, in GAT A Research Ceutre. A standart procedure was first developed to create segmental defects that did not heal and in which non-un ion developed consistently. Allogeneic DBM alone was then transplanted into these defects in one group and both allogeneic DBM and autogenous bone marraw were in the other. Healing was followed clinically, radiologically and histologically for 13 weeks in all groups. Incidence of union was 73% in allogeneic DBM implanted group alone and was 100% in both allogeneic DBMand autogenous bone marrow implanted group. Minimum time to see roentgenographically visible bone and minimum time to produce union were also shorter in the latter one. We conclude that both methods are valuable in repair of Large femoral diaphyseal defects and that allogeneic DBM and auto genous bone marrow transplantation method is more effective than allogeneic DBM implantation method alone.

Tavşanlarda uzun kemiklerdeki geniş segmental defektlerin iyileşmesinin allojenik demineralize kemik matriksi ve otojen kemik iliği transplantasyonu ile uyarılması

Abstract

Çalışmamız geniş femur diafiz defektlerinin tedavisinde allojenik demineralize kemik matriksi (DBM) ve otojen kemikliği transplantasyonunun etkisini saptamak amacıyla. GATA araştırma merkezinde, tavşanlar üzerinde gerçekleştirildi. İlk önce iyileşmeyecek ve non-unionla sonuçlanacak bir standart segmental defekt oluşturma yöntemi geliştirildi. Daha sonra ise oluşturulan bu defektlere bir grupta sadece allojenik DBM. bir başka grupta ise allojenik DBM ile birlikte otojen kemik iliği transplantasyonu yapıldı. Tüm gruplarda 13 hafta süre ile iyileşme, klinik radyolojik ve histolojik olarak takip edildi. Union insidansı sadece allojenik DBM implante edilen grupta %73, hem allojenik DBM ve hem de otojen kemik iliği implante edilen grupta ise %100 idi. Ayrıca radyolojik olarak kemik dokunun ilk kez görülmesine kadar ve union oluşmasına kadar geçen minimum süreler son grupta daha kısa idi . Böylece sonuç olarak, her iki yöntemin de geniş femoral diafiz defektlerinin tamirinde değerli olduğu ayrıca allojenik DBM ile birlikte otojen kemik iliği transplantasyonu yönteminin, sadece ollajenik DBM implantasyonu yönteminden daha etkin olduğu saptandı.
