A Biomechanical Study onA New Mini External Fixator Developed in our Clinics For Transverse Patellar Fractures

Abstract

A New Mini External Fixator (MEF) for transverse patellar fractures was developed. A model patella and the system develop ed by Weber were used to test and compare its stabylity with the conventional cerclage techniques.
Magnus on's technique, standart AO tension band wiring, AO modified tension-band wiring and the MEF were applied to the model patella. After the distal pole of the patella was fixed, traction was applied to the proximal pole. Load to failure was recorded for each technique.
Then MEF was applied to five cadaver knees with transverse patellar fractures. Stability was maintained and no incongruency was seen at a flexion arc of 90 degrees.
With the maximum load of 128.5 kgr. used in this study, incongruencies of l4 millimeters in Magnuson's techinique, 6.5 millimeters in AO modified tension-band techinique, and 1.5 millimeters in Standard AO technique were noticed. On the contrary we have not seen any incongruency in MEF we have used. These data have made us agree upon the idea that an adequate mechanical stability was provided by MEF.
We concluded that the MEF offered better stability than the other methods and this has led us to consider it for clinical use.

Transvers patella kırıkları için kliniğimizde geliştirilen mini eksternal fiksatör'e ait biyomekanik bir çalışma

Abstract

Transvers patella kırıkları için Kliniğimizde geliştirdiğimiz mini eksternal fiksatör'ün (MEF) stabilitesini, patella transvers kırıkların tedavisisinde yagın olarak kullanılan mevcut serklaj tekniklerinin stabilitesi ile kıyaslamak amacıyla Weber'in geliştirdiği bir sistem ve model patella üzerinde biyomekanik bir çalışma yapıldı.
Magnuson, AO'nun standart tansiyon bant tekniği. AO'nun modifiye tansiyon bant tekniği ile MEF, biyomekanik çalışmamızda kullandığımız sistemdeki model patella üzerine tekniğine uygun olarak adepte edildi. Patellanın distal kutbu satibt kalıp proksimal kutuptan traksiyon kuvvetleri uygulandı ve osteosentez tekniklerinin distraksiyon kuvvetlerine dayanabilirliği kendi aralarında ve MEF ile kıyaslandı.
MEF deneysel patella transvers kırığı oluşturulan 5 kadavra dizinde de denendi , dizin 0-90 derecelik fleksiyon-ekstansiyon arknda ve mükerrer hareketlerinde fragmanlarda stabilizasyonun bozulmadığı, eklem yüzünde basamaklanma olmadığı gözlendi.
Bu biyomekanik çalışmanın sonucu olarak MEF'ün model patella üzerinde diğer tekniklerden daha stabil bir osteosentez sağladığı ve patella kırıklarında uygulanabileceği düşünüldü.
