The callotasis method of limb lengthening (Experiment in dogs)

Abstract

In this study, our aim was to determine the success of extending extremity by the method of callotasis, using external fixator with ''bilateral frame'' following osteotomy and also to investigate the effect of the level of osteotomy on to the results whether applying it on the metaphysis or on diaphysis. We applied the osteotomy on 6 dogs at the level of metaphysis and on 6 dogs at the level of diaphysis. After waiting for 10 days, we have carried out distraction about 0.25 mm long in evety twelve hours throughout 40 days. We took roentgenograms at the third, fourth, seventh and tenth weeks of osteotomy. At the end of tenth week, we made macroscopic and radiographic evaluation of 12 femurs. Later, we examined six of these femurs histologicall (three of them were metaphyseal and three of them were diaphyseal) and the other six femurs biomechanically (three of them were metaphyseal and three of them were diaphyseal). The average Iength increase was 2 cm (11%) and there was no significant difference between these two osteotomies. But the roentgenographic, histological and biomechanical evaluations were more successful in the favor of the metaphyseal osteotomy. We were successful to unite all of the bones without any complications. We may suggest according to our findings which we have mentioned above, that the metaphyseal osteotomy technique will give better results than diaphyseal osteotomies on extending extremities with the method of callotasis. Furthermore we think that callotasis is a reliable method with lesser complications in the operations of extending extremities

Kallotasis metoduyla ekstremite uzatılması

Abstract

Çalışmamızda; osteotomiyi takiben ''bilateral freymli'' eksternal fiksatör kullanarak kallotasis yoluyla ekstremite uzatma işleminin başarı durumunun belirlenmesi ve bu uzatma işleminde, osteotominin metafiz ya da diafizden yapılmasının, sonuçlara etkisinin araştınlması amaçlandı. Altı köpeğe metafizer, altı köpeğe diafizer osteotomi yapılarak on gün beklendi. Daha sonra 40 gün süreyle 12 saatte bir 0.25 mm'lik kallotasis metoduyla uzatma işlemi uygulandı. Osteotomiden hemen sonra, 3, 5, 7 ve 10 haftalarda röntgen filmleri çekildi. 10 haftalık deney sonunda 12 femur önce makroskopik ve radyolojik olarak incelendi. Daha sonra bunlardan 6'51 (3'ü metafizer, 3'ü diafizer) histolojik, altısıda (3'ü metafizer, 3'ü diafizer) biyomekanik incelemeye alındı. Uzatma sonrası kazanılan ortalama uzunluk 2 cm (%11) olup, metafizer veya diafizer osteotomi yapılan gruplar arasında belirgin bir fark yoktu. Fakat rontgenografik, histolojik ve biyomekanik inceleme sonuçları, metafizer osteotomi yapılan grup lehine daha başarılı idi. Tüm osteotomilerde yeterli kaynama temin edildi ve önemli bir komplikasyon görülmedi Biz yukarıdaki bulguları gözönüne alarak, kallotasis metoduyla yapılacak olan ekstremite uzatmalarındaki; metafizer osteotomi tekniğinin diafizer osteotomilere göre daha başarılı sonuçlar vereceğini ve ekstremite uzatma ameliyatlarında kallotasis işleminin çok az komplikasyonları olan, güvenilir bir metod olduğunu düşünmekteyiz.
