Treatment of the relapsed and neglegted clubfoot with ilizarov distraction principles (preliminary report)

Abstract

On September 1989, at SSK Istanbul Hospital Orthopeadics and Traumatology Department, a 24-years-old male with neglegted clubfoot has been treated by ilizarov's distraction principles. External fixatör which applied to patient is a device designed by us according to ilizarov's concept Patient was followed up one year. At the latest control, he was able to walk with a plantigrade foot, blood circulation and temperature of foot Was equal to normal side and dorsiflexor and peroneal muscles had a good range of power Operated foot was three centimeters shorter than opposite side (28-25 centimeters) and patient had gained working ability on standing position (he works as a waiter now).
This method is non-invasive and doesn't have infection and nonunion risks; It doesn't cause trophic disorders but increases vascularity. Instead of shortening, it lengthens the foot and includes rehabilitation program during course of treatment. Those are adventages of this type of treatment
As a result, because of shortness of follow up period and only one case treated by this technique, 15 it not possible to put forward an idea now. In our clinic, there are also six patients being treated by this method.
The results of these cases will be reported later.

Tedavisi ihmal edilmiş ve nüks pes ekinovarusların İlizarov distraksiyon prensipleri ile tedavisi

Abstract

SSK İstanbul Hastanesi Ortopedi ve Travmatoloji Kliniğinde Eylül 1989 tarihinde 24 yaşında, 501 ayakta tedavi edilmemiş ileri derecede PEV deformitesi bulunan hastanın, iıizarov distraksiyon prensipleri ile tedavisi yapılmıştır. Hastaya uygulanan fiksatör, ilizarov prensiplerini temel alarak tarafımızdan tasarlanmış bir cihazdır. Olgu 1 sene müddetle takip edilmiştir. Hastamız son kontrolunda ayak tabanı ile yürüyebilmektedir. Yardımcı cihaz kullanmamakta, ayak kan dolaşımı ve ısısı sağlam tarafla eşit, dorsifleksor ve fibular kas gücü ayak fonksiyonlan için veterli güçtedir. Müdahale edilen ayak diğer taraftan yalnızca 3 cm küçüktür (28 cm-25cm) ve hasta ayakta çalışabilecek yeteneği kazanmıştır (Garsonluk yapmaktadır). Bu metod non-inveziv olup, infeksiyon riski ve pseudo-artroz riski taşımamakta ve trofik bozukluk yapmadığı gibi aksine kanlanmayı arttırmaktadır. Ayağı küçültmeyip tersine diğer ayağın boyutlarına eriştirebilmesi ve rehabilitasyon süresinin tedavi süresi içinde olması bu tedavinin avantajlı yönleri olarak görülmüştür. Sonuç olarak bu yayının ilk rapor olması ve takip süresinin çok kısa olması sebebiyle bu konuda kesin fikir yürütmek olanaksızdır. Kliniğimizde bu yöntem kullanılarak 6 olgu tedavi edilmiştir. Ancak neticeleri erken olduğu için sonuçları bilahare sunulacaktır.
