Results of conservatively treated elbow dislocations

Abstract

Twenty-six elbow dislocations treated by closed reduction were evaluated clinically and radiographically after an average of 39.8 months (18 to 55 months). Average age at the time of injury was 25.5 (range, 12 to 65 years). 9 patients had associated fractures, the most frequent being a fracture of the medial epicondyle (6 patients). The immobilization period for pure dislocations was three weeks. Associated fractures were treated surgically in three patients. Despite the generally favorable results, 62% of the patients reported some symptoms on follow-up. The most frequent residual complaints were Iimitation of motion and feeling of weakness. Flexion contracture and Iimitation of flexion were documented in 50% and 58 % of the patients, respectively. Severe Iimitation of motion was noted in 7 patients (27%). The prognosis was worse in cases with associated fractures. Limitation of motion was more severe in patients over 18 years of age. Radiographs demonstrated myositis ossificans in 19% of the patients. An adult who had an elbow dislocation associated with a capitellum fracture 17 years ago was evaluated separately. He had severe osteoarthrosis. The results of the present study indicate that elbow dislocations are not benign injuries.

Dirsek çıkıklarının konservatif tedavi sonuçları

Abstract

Kapalı olarak tedavi edilen 26 dirsek çıkığı olgusu ortalama 39.8 ay (18-55 ay) sonra klinik ve radyolojik olarak değerlendirildi. Yaralanma anında ortalama yaş 25.5 (12-65 yaş arası) idi. 9 olguda ek olarak çeşitli kırıklar da vardı. En sık gprülen ek lezyon medial epikondil kırığı idi (6 olgu). Pür çıkıkların tümünde immobilizasyon süresi 3 hafta idi. 3 olguda kapaft redüksiyondan sonra operasyon gerekti. Sonuçlar genelde iyi olmasına rağmen 16 hastada (%62) rezidüel yakınmalar vardı. En sık görülen yakınmalar hareket kısıtlılığı ve güçsüzlük hissi idi. Hastaların %50'sinde ekstansiyon, %58'inde fleksiyon kısıtlılığı saptanmakla beraber yakınmaya yol açacak derecede kısıtlılık 7 hastada (%27) mevcuttu. Kırıklı olguIarda prognozun daha kötü olduğu görüldü. 18 yaş ve üzerindeki hastalarda hareket kısıtlılığı daha ciddi idi. Radyografik incelemede olguların % 19'unda myozitis ossifikans saptandı. Ek olarak dış kondil kırığı olan ve 17 yıl sonra tesadüfen ( ayrı olarak) değerlendirilen bir erişkin olguda ileri derecede artroz saptandı. Dirsek çıkığının benign bir yaralanma olmadığı sonucuna varıldı.
