Dimon-Hughston technic of unstable trochanteric fractures; treatment and results

Abstract

The results of internal fixation and open reduction by Dimon-Hughston technic in 30 unstable trochanteric fractures, treated between 1986-1990 years at Orthopaedic and Traumatology Department of Uludag University Medical School were reported in this study. The fixation was made with 130° AO/ASIF plate under spinal or general anesthesia. To Kyle and Gustilo's criteria; we obtained 21 very good, 6 good, 2 moderate and 1 poor results in whom a deep infection was developed. ln our opinion, Dimon-Hughston metod, is a practical technic from the point of our country's condition, as it doesn 't need sophisticated tools and easy, safe method for unstable trochanteric fractures.

Stabil olmayan trokanterik kırıklarda Dimon-Hughston yöntemi ve sonuçları

Abstract

Çalışmamızda; Uludağ Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalı 'nda, 1986-1990 tarihleri arasında, 30 stabil olmayan trokanterik kırıklı hastaya Dimon-Hughston yöntemiyle yapılan açık redüksiyon ve internal tesbitin sonuçları ve bu sonuçlarda rol oynayan etkenler araştırıldı. Spinal veya genel anestezi altında, kırık, Dimon-Hughston tekniğine uygun olarak 130°’lik AO/ASIF plağı ile tesbit edilerek, Kyle ve Gustilo'nun değerlendirme kriterlerine göre 21 olguda çok iyi, 6 olguda iyi, 2 olguda orta, derin enfeksiyon gelişen bir olguda kötü sonuç saptandı. Çalışma sonucunda, Dimon-Hughston metodunun, stabil olmayan trokanterik kırıklarda, kolay, güvenilir bir metod olması, kırık masası ve skopi gibi araçlara gereksinim göstermemesi nedeniyle, ülkemiz koşullarına uygun bir yöntem olduğu yargısına varıldı.
