Arthroscopically assisted closed lateral capsuler release in treatment of patellar instability and patello-femoral syndromes in professional athletes -II-

The results of 18 professional athletes, treated by arthroscopically assisted closed leteral capsular release for patello, femoral pathologies unresponsive to 6 months conservative treatment were prospectively analyzed. There were 10 male and 8 female patients. Mean age was 23.7 and mean follow-up was 18.4 months. The diagnosis was lateral patellar hyperpression in 7 cases, patellar subluxation in 7 cases and patello-femoral arthritis in 4 cases. After a detailed clinical and three-plane radiological assesment, the patients under went a diagnostic arthroscopy followed by a closed subcutaneous lateral release. There were no early postoperative complications. The clinical results were graded according to the scoring system of Hall. They were rated excellent in 6 and good knee 1 in lateral hyperpression group; excellent in 6 and good in 1 knee in the subluxation group; good in 1, and bad in 2 knees in the arthritis group. It was concluded that arthroscopically assisted closed lateral capsular release is a simple and effective procedure when performed in athletes who meet the indication criteria.
Hacettepe Üniversitesi Ortopedi Kliniği'nde 1988-91 yılları arasında 6 aylık konservatif tedaviye cevap vermeyen patello-femoral patolojisi olan 18 profesyonel sporcuya artroskopik kontrol altında kapalı dış kapsüler gevşetme uygulandı. 10 erkek, 8 kadın sporcunun ortalama yaşları 23.7, ortalama izlem süresi 18.4 aydı. 7 olguya pateral patellar aşırı basınç sendromu, 7 olguya patellar subluksasyon ve 4 olguya patello-femoral artrit tanısı konmuştu. Detaylı klinik ve üç planda radyolojik değerlendirmelerden sonra hastalara tanısal artroskopi ve kapalı dış kapsüler gevşetme uygulandı. Erken post-operatif komplikasyona rastlanmadı. Sonuçlar, Hall skorlama sistemine göre değerlendirildi. Patellar sublüksasyonu olan 6 olguda çok iyi, bir olguda iyi sonuç; patellar aşırı basınç sendromu olan 6 olguda çok iyi, bir olguda iyi sonuç; patello-femoral artriti olan bir olguda iyi, bir olguda orta ve iki olguda kötü sonuç elde edildi. Endikasyon kriterlerine uyularak yapılan kapalı dış kapsüler gevşetmenin patello-femoral patolojilerin tedavisinde etkin, teknik olarak basit ve güvenilir bir yöntem olduğu sonucuna varıldı.
