Modified femoral neck osteotomy in chronic slipped femoral capital epiphysis (early results)

Abstract

Slipped femoral capital epiphysis, though it is rarely seen, is an important orthopedic pathology because of its complications. The treatment of slipped femoral capital epiphysis is still obscure today. In slight slips, insitu pinning method is used for treatment where as in moderate and severe slips different kinds ofosteotomies are applied. In our clinic, we treated 8 patients with moderate and severe slipped femoral capital epiphysis, by modified femoral neck osteotomy. In all cases, we used Richards dynamic compression screw as for fixation and we also placed an pedicie bone graft in osteotomy area. In our group, containing 8 patients, there were 2 cases with stiffness on the left and 6 cases with stiffness on the right. 3 of them had chronic slip where as 5 of them had acute on chronic slip. The mean age of the group was 15.5 (14-16) years and the average slipping rate was 48 (38-67) degrees. The mean follow up period of this group was 7.3 (3-13) months. After the treatment following results were obtained: 7 excellent and 1 poor results. Thus, we conclude that femoral neck osteotomies do not cause avascular necrosis and other complications as it is believed to do so and furthermore it is an effective method in providing physiology and anatomy which are the basic principles of treatment.

Kronik Femur Başı Epifizi Kaymasında Modifiye Femur Boynu Osteotomisi (erken sonuçlar)

Abstract

Nadiren görülmesine karşın komplikasyonları nedeni ile önemli bir ortopedik patoloji olan femur başı epifiz kaymasının tedavisi günümüzde tam olarak çözümlenememiştir. Hafif kaymalarda tedavi şekli insitu çivileme tarzında iken orta ve ileri derecede kaymalarda değişik tür osteotomiler uygulanmaktadır. Biz kendi kliniğimizde orta ve ileri derecede kayması bulunan 8 olguyu tarafımızdan modifiye edilen bir femur boynu osteotomisi ile tedavi ettik. Tüm vakalarımızda fiksasyon materyeli olarak Richards dinamik kompresyon çivisi kullandık, ayrıca bir kas pedikülü greftini de osteotomi bölgesine yerleştirdik. Serimizi 8 hasta oluşturmakta olup, 2 hastada sol, 6 hastada ise sağ tarafta tutulum vardı. 3 hastada kronik kayma mevcutken, 5 hasta kronik zeminde akut atakla başvurdu. Hastalarımızın ortalama yaşı 15.5 (14-16) olup, kayma miktarı ortalama 48° (38°-67°) olarak saptandı. Ameliyat ettiğimiz hastalar ortalama 7.3 (3-13) ay takip edilmiş olup, 8 hastamızdan 7'sinde çok iyi sonuç alınırken 1 hastada orta sonuç elde ettik. Sonuç olarak, femur boynu osteotomilerinin korkulduğu kadar avasküler nekroz ve diğer komplikasyonlara neden olmadığı ve tedavinin temel prensibi olan fizyoloji ve anatomiyi sağlamakta etkin bir tedavi yöntemi olduğunu düşünmekteyiz
