The resulls of CD-Instrumentation in the surgical treatment of neuromuscular scoliosis

Abstract

Scoliosis secondary to neuromuscular diseases are probably the former type of the defined spinal deformities. In addition to systemic complications such as cardiopulmoner insufficiency due to thoracal and abdominal collaps and internal organ failures, scoliotic deformity can results with pelvic obliquity and funclional disability. Neuromuscular scoliosis is progressive even after skeletal maturity. Thus, surgical treatment is inevitable in prevention of probable complications and in correction on the deformity. In this study, 12 neuromuscuIar scoliotic patients due to poliomyelitis who were treated surgically with Cotrel Dubousset lnstrumentation in the 1st Orthopaedic and Traumatology Clinic of Ankara Social Security Hospital between August 1989 and March 1991 are examined. All of the patients were female. Mean age was 14.1 (13-18) years and mean follow-up period was 24.3 (14-32) months. All patients had C type scoliosis and 6 had regular pelvic tilt. Preoperatif mean Cobb angle was 57.3° (42°-78°) and with the Cotrel-Dubousset lnstrumentation, on an average of 44.9% correction was achieved. Pelvic tilt was corrected by 63.1%. Normal physiologic lordosis was achieved at the lumber region in 50% of the patients. In sight of these data, it is suggested that Cotrel-Dubousset technique can be used successfully in the neuromuscular scoliosis secondary to poliomyelitis.

Nöromusküler Skolyozun Cerrahi Tedavisinde Cotrel-Dubousset İnstrümentasyonu Uygulaması Sonuçları

Abstract

Nöromüsküler hastalıklara sekonder skolyoz, muhtemelen tanımlanan spinal deformitelerin eski şeklidir. Skolyotik deformite, torakal ve abdominal kollapsa bağlı kardiyopulmoner yetmezlik ve iç organ bozuklukları gibi sistemik komplikasyonlar yanısıra, pevlik oblisite ve fonksiyonel maluliyete yol açabilir. Nöromusküler skolyoz iskelet malüritesinden sonra bile ilerleyici olur. Bu nedenlerle deformitenin düzeltilmesi olası komplikasyonların önlenmesi için cerrahi tedavi kaçınılmaz olur. Bu çalışmada, SSK Ankara Hastanesi, 1. Ortopedi ve Travmatoloji Kliniği'nde Ağustos 1989 ile Mart l991 tarihleri arasında Cotrel-Duboussel instrümentasyonu kullanılarak cerrahi korreksiyon yapılan 12 poliomyelit sekelli nöromusküler skolyotik hasta incelendi. Hastaların tamamı kızdı Yaş ortalaması 14. 1 (13-18 yaş) yaş idi. Ortalama takip süresi 24.3 (14-32 ay) aydı. 6 hastada regüler pelvik oblisite mevcuttu. Preoperatif ortalama Cobb açısı, 5.3° (42°-78°) olup, Cotrel-Duboussel uygulaması ile, ortalama %44.9 korreksiyon temin edildi. Pelvik oblisite %63.1 oranında düzeltildi. Lomber bölgede %66.6 hastada normal fizyolojik Iordoz oluştuğu belirlendi. Bu verilerin ışığı altında Cotrel-Dubousset tekniğinin poliomyelite sekonder nöromusküler skolyozda başarı ile kullanılabileceği görüşü elde edildi.
