Conservative theraphy method and conclusions about subacromial impingement syndrome

Abstract

Subakromial impingement syndrome is caused when acromion, corocoacromial ligament is squeezed corracoid process and/or acromioclaviculr joint during glenohumeral jonitmovemetns and especially flexions and rotations. Patients having shoulder pain diagnosed as subacromial impingement, are treated surgically or in the conscervative way according to the phase of the disease. Between the years 1990-1993, 68 patients were diagnosed as subacromial impingement at İstanbul University, İstanbul Faculty of Medicine, Clinic of Orthopaedy and Travmatology. 10 of thes patients were evaluated as phase 3 impingement and primary surgical treatment was applied. Conservative treatment was appplied to the remaining 58 patients. For 8 of the 58 patients ents who received conservative treatment, surgical treatment was applied later since the symptoms did not disappear during the treatment. 9 of the 50 patients did not continue the treatment and the remaining 41 treatments were finalized. The patients were evaluated according to the Constant Scoring before and after the conservative treatment. During the treatmennt, an increase of 36.1 points was attained in the score. Application of regular conservative treatment for the cases diagnosed as the subacromial impingement, have resulted in dimminishing of symptoms and satisfactory outcomes.

Subakromial sıkışma (impingement) sendromunda konservatif tedavi metodu ve sonuçlarımız.

Abstract

Subakromial sıkışma sendromu rotator manşet mekanizmasının glenohumeral eklem hareketleri ile özellikle fleksiyon ve rotasyonlar sırasında akromion, korakoakromial ligaman, korakoid çıkıntı ve/veya akromioklavikuler eklem arasında sıkışmasıyla oluşur. Omuz ağrısı olan ve subakromial sıkışma sendromu tanısı (SAS) konan hastalar, hastalığın evresine göre konservatif veya cerrahi olarak tedavi edilirler. 1990-1993 yıllan arasında İ.Ü. İstanbul Tıp Fakültesi Ortopedi ve Travmatoloji Kliniğinde 68 hastaya SAS tanısı konmuş, bu hastalardan 10 tanesi evre 3 impingement olarak değerlendirilip primer cerrahi tedavi uygulanmış, geri kalan 58 hasta konservatif tedavi altına altnmıştır. Konservatif tedaviye başlanan 58 hastanın, 8 tanesinde tedavi süresi içinde şikayetler geçmeyince cerrahi tedaviye dönülmüş, 50 vakanın 9 tanesi konservatif tedaviye devam etmemişler, 41 vakada konservatif tedavi sonuçlandırılmıştır. Hastalar konservatif tedavi öncesi ve sonrası Constant skorlamasına göre değerlendirilmişler, tedavi sorasında skorda 36,1 puanlık artış sağlanmıştır. SAS teşhis edilen vakalarda uygulanan düzenli konservatif tedavi sonrasında şikayetlerin gerilediği ve tedavi ile tatminkar sonuçlar alındığı saptanmıştır.
