Evulation of University of California Biomechanics Laboratory inserts in the treatment of severe flexible pes planus

Abstract

In this study, Universtiy of California Biomechanics Laboratory (UCBL) inserts were applied to fifthteen patients diagnosed with severe flexide pes planus to evaluate the effectiveness of this inserts to the orter type of shoe modifications. For this perpause, clinically observed patients, were reevaluated (balance, gait promIems and pain) while they jwere standing and walking with their UCBL inserts. After the UCBL inserts have been utilised by the patients for three months, patients were walked on the mechanical treadmill until they had the feeling of tiredness to evaluate their duration of walking and double product values which were one of the criterias of walking and double product values which were one of the criterias of energy expenditure of the body. To understand the effectiveness of the UCBL inserts in achieving the normal anatomical position of the foot, plantar flexion angle of talus and calcaneal pitch were observed on X-ray radiograms of each patient in both conditions. The collected data were analysed with statistical methods. According to our results, in a fixed time, patients were able to walk clinically much more efficient and with a Iesser energy expenditure while they were having UCBL inserts.

Pes planus tedavisinde University of California Biomechanics laboratory (UCBl) tip tabanlıkların değerlendirilmesi

Abstract

University of Cafifornia Biomechanics Laboratory (UCBL) tabanlıkların diğer tabanlık ve ortopedik ayakkabılara göre yararlarını değerlendirmek üzere yapılan bu çalışmada, pes planus tanısı konmuş olan onbeş hastaya UCBL tabanlık yapılmıştır. Klinik olarak gözlenen hastalar, UCBL tabanlığı kullanmaya başladıktan sonra tekrar değerlendirilmiş, yürürken ve ayakta dururken. denge, yürüme bozuklukları ve ağrı durumları incelenmiştir. UCBL tabanlığı üç ay boyunca kullandıktan sonra, hastalar, UCBL tabanlık ve UCBL tabanlıksız olarak mekanik treadmillde yoruluncaya kadar yürütülmüş ve yürüme süreleri ile myokard kasının oksijen tüketimini gösteren Double Product değerleri saptanmıştır. UCBL tabanlıkların, ayağın anatomik pozisyonununa olan etkisini değerlendirmek amacıyla, talusun plantar fleksiyon açısı ve calcaneusun yükseklik açısı, her hastanın UCBL tabanlıklı ve UCBL tabanlıksız olarak çekilmiş olan röntgen filmleri üzerinde incelenmiştir. Elde edilen veriler istatistiksel yöntemler ile analiz edilmiştir. Vardığımız sonuca göre. hastalar UCBL tabanlık kullanırken, aynı sürede, daha az enerji harcayarak ve klinik şikayetleri olmadan yürüyebilmektedirler
