Abstract

161 patients ( 49 female and 112 male) with an average age of 31.2 years with 173 fractured tibias (73 right and 781eft) who had sufficient x-rays to evaluate the fracture type that were treated in Ankara University Medical Faculty, Ibn-i Sina Hospital, Department of Orthopaedic Surgery and Traumatology between January 1987 and January 1992 were called for follow-up examination to evaILİate the results and probable corhplications. 141 of the patients had isolated tibia fractures while 32 had additional skeletal fractures. One pateint had an amputation and one patient died due to complications following treatment. 89 patients were Iost to followup. The average time required for complete fracture healing was 34.9 weeks in the conservatively treated group, 41.7 weeks with the externalfixator and 50.9 weeks in the operatively treated group (plate fixation). When the results were evaluated according to the fracture type using AO classification; the fractures in Group B healed faster when conservatively treated compared to the surgical treatment group with a statistically significant difference (p<0.05).

Abstract

Ankara Üniversitesi Tıp Faküftesi ibn-i Sina Hastanesine Ocak 1987 ile Ocak 1992 tarihleri arasında müracaat etmiş ve film arşivinde kaza sonrası kırık tipinin anlaşılabileceği yeterli filmi olan 49'u kadın, 112'si erkek, ortalama 31~2 yaşında 161 hastanın 73'ü sağ ve 78'i sol bacağa Iokalize 173 tibia kırığı değerlendirmeye alındı. Tibia kırıklarının 141'i izole olup, 32'sinde ek kırıklar mevcuttu. Hastalar tedavi metoduna göre iyileşme süreleri ve komplikasyonları değerlendirmek üzere kontrole çağrıldı. Kontrole gelen 72 hastanın ortalama takip süresi 30.07 ay idi. Hastalardan kırıkları nedeni ile ampute olan ve eks olan iki hasta çalışmaya alınmadı. Ortalama iyileşme süresi konservatif tedavi edilenlerde 34.9 hafta, eksternal fiksatörle tedavi edilenlerde 41. 7 hafta, plaklanarak uygulanan cerrahi tedavide ise 50.9 hafta olarak bulundu. AO klasifikasyonuna göre alt gruplara ayrıldığında B grubunun konservatif tedavisinin cerrahiye göre üstünlüğü p<0.05 seviyesinde anlamlı bulundu.
