False meniscal tears in magnetic resonance imaging of the knee (Comparison with arthroscopic findings)

Abstract

Magnetic resonance imaging (MRI) of the knee followed by an arthroscopic procedure was performed in 80 patients between March 1992-November 1993. There were 57 male and 23 female patients and the mean age was 33 (16-64). Based on arthroscopic findings the sensitivity, specificity and accuracy of MRI was 97.1%, 60%, 76.3% for medial meniscal tears and 60%, 88%, and 81.3% for Iateral meniscal tears respectively. False positive results were more common on the medial side {18), than the Iateral side (7). False positive medial tears were all reported to be in the posterior horn while false positive Iateral tears were reported to be in the anterior or posterior horns. Of all the cases with a false positive report, only two patients had a completely normal arthroscopic examination while the rest had other intra-articular pathologies {mostly chondrallesions) to account for their symptoms. There were 8 false negative results on the Iateral side and one false negative result onthe medial side. Two of these Iateral false negative tears involved the posterior horn, three were transverse tears near the Dopliteal hiatus, one was an incomplete peripheral tear, one was a complete peripheral seperation and one was a torn discoid meniscus. The false negative medial tear involved the posterior horn. It was concluded that, a meticulous clinical examination followed by an arthroscopic procedure is the gold standard in the diagnosis and treatment of meniscallesions. Given the high cost and relatively high false results, MRI should be reserved for cases with tumoral conditions, high Ievel injured athletes in ''immediate need to know'' situations and patients with chronic symptoms that present with a difficulty in diagnosis.

Dizin manyetik rezonans görüntülemesinde yalancı menisküs yırtıkları (Artroskopi buıguları ile karşılaştırma)

Abstract

Mart 1992-Kasım 1993 tarihleri arasında SSK Ankara Hastanesi 1. ve 2. Ortopedi ve Travmatoloji kliniklerinde 80 hastaya diz manyetik rezonans görüntülemesi {MRG) sonrası artroskopik giriş uygulandı. Hastaların 57'si erkek, 23'ü kadındı ve ortalama yaş 33 {16-64) olarak bulundu. Artroskopi bulguları temel alınarak MRG'nin medial menisküs yırtıklarındaki duyarlılığı %97.1, özgüllüğü %60 ve doğruluğu %76.3 olarak bulundu. Aynı değerler Iateral menisküs yırtıkları için sırasıyla %60, %88 ve %81.3 olarak hesaplandı. Yalancı pozitif yırtıklar medial tarafta Iaterale göre 2.5 misli fazla olarak görüldü. Yalancı pozitif yırtıklar menisküs için hep arka boynuzda, Iateral menisküs içinse hem ön hem arka boynuzda rapor edilmişti. Medial ve Iateral yalancı pozitif sonuç rapor edilen hastaların ikisi hariç hepsinde semptomlarının açıklayacak başka eklem içi Iezyonlan vardı, 2 olguda ise artroskopik inceleme tamamen normaldi. Lateral tarafta 8 olguda yalancı negatif sonuç rapor edildi. Artroskopide bu hastaların 2 tanesinde arka boynuz yırtığı, 3'ünde popliteal hiatus seviyesinde transvers yırtık,1'inde inkomplet periferik yırtık, 1'inde tam periferik ayrılma ve l'inde yırtık diskoid menisküs saptandı. Medial tarafta bir yalancı negatif sonuca rastlandı. Dikkatli bir klinik inceleme sonrası yapılacak artroskopinin menisküs yırtıklarının tam ve tedavisinde ''altın standart'' olduğu MRG'nin pahalı olması ve oldukça önemli oranlarda yanıltıcı sonuç vermesi nedeniyle, acil tanı konması gereken yüksek düzey sporcular, tam güçlüğü olan kronik semptomatik dizler ve dizin menisküs dışı patolojilerinde kullanılması gereken bir yöntem olduğu sonucuna varıldı.
