Primary open reduction of developmental dislocation of the hip through medial approach

Abstract

From February 1985 to December 1990, twenty-eight developmental dislocated hips were treated by open reduction through a medial approach. The average follow-up was 64.4 months. There were 18 females (85.7°%) and 3 males (14.3%). Mean age at surgery was 12.9 months (range 7-19 months). No avascular necrosis was seen. 5 hips (17.1 %) have already undergone or 10 of them will undergo subsequent surgery because of acetabular dysplasia or subluxation of the femoral head.

Abstract

Şubat 1985-Aralık 1990 tarihleri arasında SSK İstanbul Hastanesi I. Ortopedi ve Travmatoloji Kliniğinde gelişimsel kalça çıkığı (GKÇ) olan 21 çocugun 28 kalçası, medial girişim ile açık redüksiyon (M.A.O.R) yöntemi ile tedavi edildi. 18'i kız (%85.7), 3'ü erkek (%14.3) olan olgularımızın yaşları, en düşük 7 ay, en yüksek 19 ay olmak üzere ortalama 12.9 aydır. Olgularımızın en az 36 ay, en çok 110 ay olmak üzere ortalama 64.4 ay (=5.3 yıl) takip ettik. Ameliyat öncesi disloke kalçaların asetabular indeksleri (Ai) ortalama 37.7° ve yapılan son kontrollerde 21.2° olarak bulundu. Yaptığımız son kontrolde, MAOR yöntemi ile tedavi edilen 28 kalçayı Severin radyolojik kriterlerine göre değerlendirdik ve 12 kalçayı (%42.8) Grade I, 6 kalçayı (%21.4) Grade II, 7 kalçayı (%25) Grade III ve 3 kalçayı (%10.7) Grade IV olarak sınıflandırdık. Olgularımızın yapılan son fizik muayenelerinde ise modifiye McKay kriterlerini kullandık ve 25 kalçayı (%89.2) çok iyi ve 3 kalçayı (% 10.7) iyi olarak bulduk. 28 kalçanın 5'i (% 17.8) için ek cerrahi girişim gerekti. Olgularımızın hiçbirinde avasküler nekroz komplikasyonuna rastlanmadı.
