The effect of segmental spinal instrumentation without fusion on longitudinal spinal growth

Abstract

In this study, the preservation of longitudinal vertebral growth was investigated by the application of interspinous segmental spinal instrumentation for eight segments without arthrodesis on 5 lambs (Group 1). Their same sex twins were selected as a control group (Group 2). Surgical procedures were performed on eight weeks of age male lambs. Both group were followed in the natural environment for seven months. Radiographic measurement of the length of instrumented vertebral segments were performed every months. The average gain in spinal length for eight instrumented segments from before surgery to final follow-up was 36.6 mm. In Group 1 and the average longitudinal growth for the same segments was 85 mm in Group 2. Spontaneous fusion between spinous processes occured in all 5 lambs. Average %43 of the growth potential was preserved with segmental spinal instrumentation without fusion. The lambs provided a successful in vivo biological model for the observation of progressive vertebral growth.

Füzyon yapılmaksızın uygulanan interspinoz segmenter spinal enstrumentasyonun omurga uzamasına etkisi

Abstract

Bu çalışmada: 5 erkek kuzu üzerinde, füzyon yapılmaksızın 8 segmente uygulanan interspinoz segmenter enstrumentasyonun omurganın uzama potansiyelini ne ölçüde etkilediği araştırıldı. Kontrol grubunu bu kuzulann hemcins ikizleri oluşturdu. Kuzular 8 haftalıkken opere edildikten sonra 7 ay süre ile doğal ortamlarında ikizleri ile beraber izlendi. Ölçümler aylık periodlarla radyolojik olarak yapıldı. Deney grubunda, enstrumentasyon uygulanan 8 segmentte ortalama olarak 36.6 mm'lik bir uzama elde edilirken, kontrol grubunda aynı segmentler için ölçülen ortalama uzama miktarı 85 mm idi. Opere edilen kuzuların hepsinde interspinoz sahada spontan füzyon gelişti. Deney grubunda omurganın, büyüme potansiyelini enstrümentasyon uygulanan segmentler için ortalama %43 oramnda koruduğu saptandı. Kuzuların bu çalışma için iyi bir invivo biyolojik model olduğu görüldü.
