Correlation between the Ievel of neurologic injury and spinal canal narrowing in burst fractures of spine

Abstract

The documents with their computerized tomographic results of forty patients who have the type of burst fractures form a total number of 128 patients treated for spinal fractures in Kartal Education and Research Hospital between 1989-1994 have been reevaluated. It is asked whether there is correlation between narrowing of spinal canal sagital diameter and neurological status of the patient. Neurological Frankel level and percentage of spinal canal narrowing of each patient has been determined and their statical data calculated with the regression analysis method. Sixteen of forty patients had neurologic injury. In five different neurologic Ievels from Frankel A to E, the mean percentage of spinal canal narrowing were 57, 80, 73, 44 and 23 respectively. It has been found there is a significant correlation between traumatic spinal canal stenosis and severity of neurologic injury. Neurologic deficit is primarily determined by the severity of the first trauma. Together with the anatomic Ievel of injury, also the degree of spinal canal stenosis is an important determining factor of the severity of neurologic injury.

Omurga burst kırıklarında spinal kanal daralması ile nörolojik yaralanmanın ağırlığı arasındaki ilişki

Abstract

Kartal Eğitim ve Araştırma Hastanesinde 1989-1994 yılları arasında tedavi edilmiş 128 omurga kırıklı hastadan burst tipi kırığı olan ve bilgisayarlı tomografik tetkikleri yapılmış 40 hastanın kayıtları yeniden incelendi. Omurga kanalındaki sagital çap daralma yüzdesi ile hastaların Frankel derecelendirmesine göre belirlenen nörolojik durumları arasında bir ilişki olup olmadığı araştırıldı. Her hastadaki daralma yüzdeleri tek tek belirlenip regresyon analizi yöntemiyle istatistiksel hesaplaması yapıldı. 40 hastanın 16'sında nörolojik yaralanma mevcuttu. Frankel A 'dan Frankel E'ye kadar beş ayrı nörolojik derecede ortalama kanal darlıkları sırasıyla 57, 80, 73, 44, 23 idi. Kanal darlığının artması ile nörolojik yaralanmanın ağırlaşması arasında doğru orantılı ve anlamlı bir ilişki bulundu. Nörolojik defisit primer olarak ilk travmanın ağırlığı ile belirlenmektedir. Yaralanmanın seviyesiyle birlikte, bu ilk travma anında oluşan kanal daralmasının miktarı da, nörolojik yaralanma ağırlığının belirleyici unsurlarındandır.
