Acute osteochondral fracture of lateral femoral condyle in karate

Abstract

Karate, meaning "empty hand" is a weaponless martial art which has originated from Japan and China it is a sport which has unique clothing and which is done on a polyurethane mattress. With in Iimits of certain ruIes, it is allowed to hit the opponent with kick and punch without giving any harm. In this report, a male karate player, who is 18 year 5 old, training Mawashi-geri technique with his right foot, forced his Ieft Iower extremity into valgus and external rotation, from his knee joint and an acute osteochondral fracture of the Iateral condyle of the Ieft femur, is presented. The fracture which first detected in plain graphies and then whose origin was seen by MRI, was treated with absorbable pins and fibrin adhesive system under open reduction. After revising the Iiteratures it is seen that fractures are misdiagnosed rather than being rare fractures. In order not to miss these fractures, after trauma to the knee when blood is aspirated from the knee with fat globules and when no other pathology is observed a through and careful investigation of the patient in terms of acute osteochondral fractures must be done.

Abstract

"Boş el" anlamına gelen ve Japonya ile Çin kökenli olan karate silahsız mücadele sanatlarından biridir. Kendine özgü kıyafetiyle, esnek, poliüretan minderle kaplı zemin üzerinde yapılan bir spordur. Kurallar dahiIinde, rakibe zarar vermeden tekme ve yumruk şeklinde yapılan ataklar serbesttir. Bu çalışmada, antreman esnasında sağ ayağıyla Mawashi-geri (dairevi tekme) tekniğini çalışan 18 yaşında erkek karatecide, yere bastığı sol alt ekstremitesinin dizinde valgus ve rotasyon ekstern zorlaması sonucu sol femur Iateral kondilinde oluşan akut osteokondral kırık olgusu ele alındı. Direkt grafide görülen ve MRl'de Iezyonun orjini saptanan olguya açık redüksiyon, fragmanın fibrin yapıştırıcı ve absorbabl pinlerle tespiti uygulandı. Literatür bilgisiyle, bu kırıkların az rastlanan kırıklar olmaktan ziyade atlanabilir kırıklar olduğu belirlendi. Bu kırıkları atlamamak için, dize gelen travma sonrası hemartroz gelişen ve ponksiyonda yağ globülleri belirlenen olgularda, başka bir patoloji saptanmadığında, akut osteokondral kırık yönünden dikkatli bir inceleme gerektiği sonucuna varıldı.
