Reverse radial forearm flap for doxorubicin induced extravasation injuries

Abstract

The extravascular escape of intravenously administered doxorubicin leads to a painful, slowly enlarging inflammatory reaction which, if not diagnosed, will progress to a chronic severe cellulitis, ulceration of the skin and possible further involvement. Two patients who underwent anti-neoplastic therapy with doxorubicin suffered extravasation injuries leading to deep tissue necrosis and exposed tendons on the dorsum of their hands. ln both cases, all the necrotic tissues were removed and the defects were covered with reverse radial forearm fIaps The results were found to be functional and aesthetic.

Abstract

İntravenöz uygulanan doksorubisinin damar dışına ekstravasyonu ağrılı, yavaşça ilerleyen inflamatuar reaksiyona sebep olmakta ve gerekli önlemler alınmazsa kronik selülit, deri ve yumuşak dokuların ülserasyonu gibi ciddi problemlere yol açabilmektedir. Kliniğimizde anti-neoplastik amaçla doksorubisine bağlı ekstravazasyon yaralanması sonucunda el dorsumunda deri ve deri altı nekrozu ile tendon ekspozisyonu gelişen iki hasta tedavi edilmiştir. Her iki vakada nekrotik dokuların debridmanından sonra oluşan defekt "reverse" radial önkol flebi ile onarılmış, sonuçlar fonksiyonel ve estetik bulunmuştur.
