Abstract
Osteoblastoma is a benign tumor the cells of which tend to differentiate in osteoblasts. Although it is debatable, aggressive forms of this tumor are also observed. We present a 52 year old female patient with 4th thoracal vertebral erosion and with medullary pressure. Histopathological examination of corpusectomy material has revealed findings consistent with aggressive osteoblastoma, because of the presence of atypical epitheloid osteoblasts. In addition to this morphology, early recurrence was observed due to the aggressive nature of the tumor and difficult resection in vertebral location.

Abstract
Osteoblastom, osteoblastlardan yapılı selim bir tümördür. Bazı yazarlarca in-situ osteosarkom ya da malign osteoblastom olarak da adlandırılan, ancak tartışmalı kabul edilen agressif gidişli formları da mevcuttur. Morfolojisi ve klinik seyri ile agressif osteoblastom olarak yorumladığımız bir olgu sunulmuştur. 52 yaşında kadın hastaya, T4 vertebra korpusunda erozyon ve medulla basısı saptanarak sağ torakotomi ile T4 korpusektomi ve füzyon uygulanmıştır. Histopatolojik incelemede tipik osteoblastom alanları yanı sıra iri, hiperkromatik nüveli. kısmen epiteloid özellik kazanmış osteoblastlardan yapılı alanlar ve erken dönemde lokal nüks nedeni ile vaka agressif osteoblastom olarak yorumlanmıştır. Hastamızda görülen erken nüksün lokalizasyon nedeni ile rezeksiyonun yeterli yapılamayışı ve agressif osteoblastomun kendine özgü seyrine bağlı olduğu düşünülmüştür. Bu tür vakaların osteoblastom benzeri düşük gradeli osteosarkomdan ayrımı güçlük oluşturabilmekte ise de bazı yazarlara göre tedavi yaklaşımları ve klinik gidiş çok farklı değildir.
