Oblique metatarsal osteotomy and its combination with the soft tissue procedures in the treatment of hallux valgus

Abstract

Between 1989-1991 in Adana Military Hospital and between 1991-1994 in Taksim state Hospital, distal metatarsal osteotomy was performed in 28 patients who had been diagnosed to have hallux valgus. Intraarticular lateral release and medial capsuloraphy were combined with the oblique metatarsal osteotomy in 18 patients if the hallux valgus angle was greater than 35 degrees. The mean age of the patients was 32.7 (20-48) and the mean follow-up was 32 months. While the preoperative hallux valgus angle and the intermetatarsal angle were 36 degrees and 14.2 degrees respectively, they were decreased to 10 degrees and 9.3 degrees postoperatively. Pain, which was the most common indication for the operation, disappeared in 97% of the patients and the cosmetic appearance of the hallux was good in all of the patients. Under the light of these findings, we think that distal metatarsal 30 degrees oblique osteotomy is not only a method of correcting the deformity in moderate hallux valgus, but also can easily be performed with a low complication rate in severe hallux valgus if combined with intraarticular lateral release and medial capsuloraphy.

Halluks valgusda distal metatarsal oblik osteotomi ve yumuşak doku girişimi ile kombinasyonu

Abstract

1989-1991 yılları arasında Adana Asker Hastanesi, 1991-1994 yılları arasında da Taksim Devlet Hastanesi Ortopedi Kliniğine başvuran 28 hastaya halluks valgus tanışı konularak distal metatarsal oblik osteotomi uygulandı. Halluks valgus açışının 35°'nin üzerinde olduğu 18 vakaya distal metatarsal oblik osteotomi ile birlikte intraartiküler lateral gevşetme ve medial kapsülorafi eklendi. Hastalarımızin yaş ortalaması 32.7 (20-48), ortalama takip süresi 32 ay idi. Preoperatif dönemde ortalama halluks valgus açışı 36° iken postop. 10°, intermetatarsal açıysa 14.2 dereceyken; postop 9.3° olarak bulunmuştur. Preoperatif dönemde en önemli ameliyat endikasyonunu oluşturan ağrı yakınımı ameliyat sonrası %97 hastada tamamen kayboldu. Ameliyat sonrası kozmetik görünüm hastalanmızın hepsi için iyi idi. Bu değerlendirmelerimiz ışığında, 30°'lik distal oblik metatarsal osteotominin orta derecedeki halluks valgus vakaları için kullanımı ideal bir cerrahi teknik olmasının yanında; aynı zamanda yumuşak doku girişimimizle kombine edildiğinde ileri dereceli halluks valgus vakaları içinde kullanılabilecek, komplikasyon oranı düşük ve son derece kolay uygulanabilir bir teknik olduğu görüşündeyiz.
