Abstract
In this study, we evaluated the result of dynamic axial fixator (Orthofix) in the treatment of 52 patients with femoral fractures that were followed on an average 39 months (min 14 months). Twelve of the fractures (23%) were closed, and 40 (77%) were open. Nine (17%) femoral fractures were stable and 43 (83%) were unstable type. In 2 cases of them dynamic axial fixator which were applied minimal osteosynthesis and were not given dynamic loading. Fifty cases with stable fractures on an average of 4 weeks and unstable fractures 7.5 weeks were given dynamic loading. On an average complete healing and removal of fixator was 28 (20-38) weeks. As minor complications 16 (6.5%) Schanz screw bending and 10 (4%) pin loosening occured. Pin tract infection rate was 19%. Seven (14.5%) of the 48 ball and socket type Orthofix have shown mechanical insufficiency and those were replaced with the new ones. In 12 out 52 of femur fractures observed major complications. Such as 11 (21%) cases had malunion, 5 (10%) had shortening (>1 cm), 5 (10%) unable weight bearing completely, 7 (13%) had knee motion restriction, and 1 (2%) had nonunion. According to this treatment the success rate was 77% (40 patients). As a result we can say that, Orthofix is a useful tool in the treatment of certain femur fractures. In order to avoid mechanical insufficiency it is advisable to prefer lengthening type Orthofix. In addition to above mentioned points, fasciotomy of fascia lata, restoration of articular surface in intraarticular fractures, and strict follow-up and rehabilitation will help to achieve better results.

Bu çalışmada dinamik aksiyel fiksatör (Orthofix) kullanılarak tedavi edilen ve ortalama 39 ay (en kısa 14 ay) süre ile izlemi yapılan 52 femur kırıklı olgu değerlendirilmiştir. Femur kırıklarının 12'si (%23) kapalı tip kırkken, 40'ı %77) açık tip kırıktı. 9 (%17) femur kırığı stabil tip kırıkken, 43'ü (%83) stabil olmayan tip kırıktı. 2 olguya orthofixle birlikte minimal osteosentez de uygulanmıştı. Bu 2 olguya dinamik yüklenme verilmedi. Diğer 50 olgunun stabil kırıklı oranlarına ortalama 4 haftada, stabil olmayanlarına da ortalama 7.5 haftada dinamik yüklenme verildi. Olguların tam kaynama ve cihazı çıkarma süresi ortalama 28 hafta (20-38 hafta) idi. Minör komplikasyon olarak 16 (%6.5) şanz vidasında eğilme ve 10 (%4) şanz vidasında da gevşeme meydana geldi. Çivi yolu enfeksiyon oranı %19'du. 48 oynar başlı orthofix'in 7'sinde (%14.5) mekanik yetersizlik oluştu ve başka bir orthofixle değiştirildi. 52 femur kırığının 12'sinde majör komplikasyonlarla karşılaşıldı. Bunlar: 11 (%21) olguda malunion. 5 (%l0) ekstremitede 1 cm'den fazla kısalık, 5 (%10) olguda tam yük verememe, 7 (%l3) olguda diz hareketlerinde kısıtlılık ve 1 (%2) olguda nonunion sorunu şeklindeydi. Bu sonuçara göre tedavisi başarıyla sonuçlanan ekstremite saysı 40 olup tedavi başarı oranı %77 olarak bulundu. Sorunlu femur kırıklarının tedavisinde orthofix önerilmekte, alette mekanik yetersizlik oluşmasını önlemek için; uzatma tipi orthofix kullanmanın, diz ekleminde kısıtlılık oluşumunu önlemek için: her olguda fasiya lataya fasiyotomi uygulamanın, eklemi içine alan kırıklarda eklem restorasyonunu tam sağlamanın ve çok iyi hasta izlemi ve rehabilitasyonu uygulamanın tedavi başarı oranını yükselteceği sonucuna varıldı.
