Arthroscopic treatment of synovial chondromatosis of the knee

Abstract

Between 1983-1994, in 11 cases, synovial chondromatosis was diagnosed and treated arthroscopically in two distinct clinics of Germany. 6 of the patients (54.5%) were male and 5 patients (45.5%) were female. Their ages varied between 22 and 41. All patients had a history of previous trauma, swelling and pain around the knee. In all cases, there were limitation of the range of motion and in 4 cases (36.3%) locking history of knee was impressing. The plain radiographs were found to be normal in all cases. During the arthroscopic examination, cartilaginous loose bodies were observed. Synovial membrane was thickened, rough and hyperemic. These loose bodies were extracted and synoviectomy was done arthroscopically in all cases. After arthroscopy no complication was observed and the patients were mobilized on the first postoperative day. The patients were followed 1-7 years (average; 4 years and 3 months). In the postoperative evaluation, complaints of the patients were disappeared. In neither of the cases recurrence was seen nor a second operation was necessary.

Abstract

Almanyada 1983-1994 yılları arasında iki ayrı klinikte toplam 11 hastaya artroskopik olarak diz sinoviaf kondromatozisi tanışı konuldu ve hastalar aynı seansta yine artroskopik olarak tedavi edildi. Hastaların hepsinde önceden geçirilmiş travma öyküsü vardı. Başlıca klinik bulgular; diz çevresinde ağrı. şişlik, hareket kısıtlılığı iken, 4 hasta (%36.3) zaman, zaman dizlerinde kilitlenme olduğundan yakınıyordu. Klasik radyolojik inceleme sonuçları normal olarak bildirilen hastalara tanı ve tedavi amaçlı artroskopik girişim uygulandığında; eklem içinde kıkırdak yapısında serbest cisimcikler, sinoviada hiperemi ve kabalaşma görüldü. Artroskopik olarak serbest cisimcikler çıkarılıp, sinoviektomi uygulandı. Ameliyat sonrası 1-7 yıl arası değişen izlem süresi içinde, hastaların yakınmalanının kaybolduğu ve nüks görülmediği belirlendi.
