Clinical experiences in avulsion injuries of the extremities

Abstract

Avulsion injuries are part of severe crush-avulsion trauma and are associated with high morbidity and mortality. The common goal of all surgical approaches is rapid re-establishment of skin coverage over injured areas. A 2-year review of experience with degloving revealed 6 patients who had sustained this trauma. All of these patients had common type degloving injury. In our clinical cases, we have not excised the avulsed flap, instead we have excised subcutaneous tissue and obtained full thickness skin graft. So, we have obtained graft from the avulsed flap. In all our cases we have successful results.

Ekstremitelerin avülsiyon yaralanmalannda klinik deneyimlerimiz

Abstract

Ekstremitelerin avülsiyon tarzındaki yaralanmaları ciddi bir ezilme-parçalanma tipi yaralanmadır ve önemli morbidite ile sonuçlanabilir. Tüm cerrahi yaklaşımlarda esas amaç, yaralı bölgenin kaybolan cilt örtüsünü en kısa sürede yeniden sağlamaktır. Bu çalışmada, kliniğine avülsiyon yaralanmasi ile başvuran 6 hasta üzerinde-ki klinik deneyimlerimiz aktarıldı. Tüm hastalarda Yaygın (common) tip denen ve açık bir yaranın eşlik ettiği avülsiyon yaralanması mevcutlu. Biz vakalanmızda avulse flebi eksize etmek yerine, subkutanöz yağ dokusunu eksize ederek onu tam kalınlıkta deri grefti haline getirdik. Elde ettiğimiz bu deri grefti ile avulse alana yeni bir örtü temin ettik. Tüm vakalanmızda oldukça başarılı sonuçlar elde ettik.
