Evaluation of 126 cases of cerebral palsy

Abstract

In this study, 126 cases of cerebral palsy were evaluated in a "spastic-handicapped rehabilitation center". The goal was to document the most frequently seen orthopedic findings in this population of cerebral palsy patients and to review the factors preventing the optimal orthopedic approach to this entity. Physical examination findings, extremity involvement, type of motor disorders and functional and therapeutic capacities were investigated. Distribution and ratios of the deformities were similar to the literature. In neurophysiologic evaluation a great deal of patients were spastic (71,4%) and anatomic location of the disease was diplegic in 57.9%. Efficacy and realization rates of treatment and follow-up were also reviewed. Treatment of hip instabilities and spinal deformities were found to be inadequate. Although surgery and/or orthotic treatment was designed for the majority of the patients, these treatment modalities could be able to be performed in only a little group because of social and economic factors. A high rate of recurrence after corrective surgery was found because of lacking of sufficient postoperative orthotic treatment.

Abstract

Bu çalışmada, bir "spastik özürlüler rehabilitasyon merkezinde ortopedi konsültasyonu yapılan 126 serebral felçli olgu değerlendirildi. Olguların fizik muayene bulguları, ekstremite tutulumları ve hareket bozukluğunun tipi, fonksiyonel ve terapötik kapasiteleri incelendi. Hastaların büyük çoğunluğu ilgili literatürle uyumlu olarak spastik ve diplejik idi. Buna bağlı olarak deformiteler de alt ekstremitelerde yoğunlaşmıştı. Hastaların takip ve tedavilerinin etkinliği ile uygulanma oranları, gözden geçirildiğinde özellikle katça instabiliteleri ve omurga eğriliklerine yönelik tetkik ve tedavilerin yeterli yapılmadığı saptandı. Hastaların çoğunluğuna cerrahi tedavi ve/veya ortez önerildiği halde bu tedaviler, genellikle sosyal ve ekonomik etkenlere bağlı olarak, sadece küçük bir grupta gerçekleştirilebildi.
