Response of growth plate to trauma and metallic implants in a rabbit model

Abstract

The aim of this study is to analyze the importance of the type of the implant, used in epiphysis fractures. For this purpose, immature New Zealand rabbits were divided into two groups. In the first group, Salter-Harris type IV epiphysis fracture was surgically produced at the right distal femoral epiphysis, then reduced anatomically and fixed with two parallel 1 mm. diameter Kirschner wires crossing the growth plate. In the second group two parallel Kirschner wires crossing the growth plate were placed at the right femoral distal epiphysis. Intact left distal femurs of the second group were used as control group. Rabbits were killed at 3 rd and 6 th weeks postoperatively. During the histopathologic examination no significant difference was found between the Kirschner and control groups. Narrowing of the growth plate and hemorrhage were the findings observed in the fracture group. Kirschner wires were thought to be more advantageous than the screws or threaded pins in the surgical treatment of epiphysis fractures. It was concluded that, the damage observed after the surgical treatment of epiphyseal fractures was primarily due to the fracture itself, if an appropriate implant had been used.

Büyüme plağının travmaya ve metalik implantlara cevabinin immatür tavşan modelinde incelenmesi

Abstract

Bu çalışmanın amacı büyük problem teşkil eden büyüme plağı zedelenmelerinin tedavisinde önemli yer teşkil eden implant seçimine değinmektir. Bu amaçla immatür beyaz Yeni Zelenda tavşanlar: ikı gruba ayrılarak birinci gruptaki tavşanların sağ femoral distal epifizinde latrojenik Salter-Harris tip 4 Kırığı oluşturuldu ve anatomik olarak redükte edilerek fizisi çaprazlayan biribirine paralel iki adet 1 mm'lik Kirschner teli ile tespit edildi. İkinci grupta ise tavşanların sağ femoral distal epifizlerine kırık oluşturulmadan, fizisi geçen birbirine par-elel iki adet 1 mm'lik Kirschner teli yerleştirildi. Kontrol grubu olarak kırık oluşturulmayan ikinci gruptaki tavşanların sol femoral distal fizisi kullanıldı. Tavşanlar 3. ve 6. haftada öldürücüler. Yapılan histopatolojik in-celemede Kirschner grubunun kontrol grubundan farkı görülmedi. Kırık grubunca ise büyüme planında daralma ve yer yer kanama odakları görüldü. Kompresyon vidası ya da yivli Kirschner yerine ince düz kirschner telleri kullanmanın daha avantajlı olduğu düşünüldü. Cerrahi olarak tedavi edilen epifiz kırıkları sonrası büyüme plağında görülen hasarın uygun implant kullanıldığı takdirde öncelikle kırığın kendisine ait olduğu sonucuna varıldı.
