Treatment of femur segmentary comminuted fractures by indirect reduction

Abstract

Femur segmentary comminuted fractures, occurring by high-power trauma, is one of the most important matters that still disturb traumatologues at present due to the difficulties of its treatment and high rate complication. In this study, we treated 16 patients with traumatic femur segmentary fractures by long condylar plate (A of 95¡), using indirect reduction method. Of our cases, 10 were wounded by traffic accident, 6 by firearms. Through the analysis of the result, it was observed that consolidation developed in all cases except one, in whom post operative infection developed and who was accepted as an successful, in 6 months on average, and at the end of this period, almost full weight was given. We think that this method, for today, may be an alternative against intramedullary locking pins which are recommended for femur segmentary fractures and accepted as a good treatment method is fairly easy, and scopy and fracture table are not needed in operation. In addition, it does not require further surgical instrumentation; another advantage of the method is that it is more economical.

Femur segmenter parçalı kırıkların indirekt redüksiyon ile tedavisi

Abstract

Yüksek enerjili travmalarla oluşan femur segmenter parçalı kırıkları, tedavisindeki zorluğun yanında komplikasyon oranının yüksekliğiyle de halen günümüzde travmatologları uğraştıran en önemli konulardan biridir. Bu çalışmamada travmatik femur segmenter kırığı olan 16 hastamızı uzun kondil plağı (95°lik AO) ile indirekt redaksiyon yöntemi kullanarak tedavi ettik. Olgularımızın 10'u trafik kazası, 6'sı ateşli silah yaralanmasıydı. Sonuçları incelediğimizde, postoperatif enfeksiyon gelişen ve başarısız kabul edilen 1 olgu dı-şında ortalama 8 ayda tüm hastalarda kaynama gelişti ve bu süre sonunda yakın yük verildi. Bugün için bu yöntemin, femur segmenter kırıkları için önerilen ve iyi bir tedavi yöntemi olarak kabul edilen kilitli intramedüller çivilere alternatif olabileceğini düşünmekteyiz. Çünkü bu yöntemin tekniği kolay olup, operas-yonda skopi ve kırık masasına ihtiyaç duyulmaz, ayrıca fazla miktarda cerrahi enstürmentasyon gerektirmez, daha ekonomik olması diğer bir avantajdır.
