Os acromiale represents a failure of fusion of the acromial ossification centers. However, often missed in the diagnostic work-up , it is usually diagnosed on an axillary plain radiograph. Os acromiale is one of the rare causes of shoulder impingement syndrome. Since it is a failure of union, its treatment emerges some problems. Primary excision of large fragments can cause catastrophic results. Here reported is a new technique done on a case with the review of the literature.
