Treatment of tibial bone defects with distraction osteogenesis

Abstract

Treatment of fractures with bone defect of the tibia is not only a challenge but also an experience for orthopaedic surgeons. 43 tibia of 41 cases with tibial bone defects were treated by distraction osteogenesis method in our clinic between January 1989 and July 1996. Age range was between 19 and 36 (mean 24.8) Cases were radiologically followed up weekly for the first month and monthly thereafter with an average of 40.7 months. Although most cases were traumatised with a high energy gunshot injury mostly requiring amputation, we succesfully treated all the cases with distraction osteogenesis. We concluded that this is an effective and alternative method to other treatment modalities.

Tibia kemik defektlerinin distraksiyon osteogenesisi yöntemi ile tedavisi

Abstract

Kemik defektlerinin tedavisi oldukça güç ve tartışmalı bir konudur. Ortopedik cerrah için defektin teda-visi önemli bir tecrübedir. Bu çalışmada kliniğimizde Ocak 1989 Temmuz 1996 yılları arasında toplam 41 olgunun 43 tibiasında kemik defektinin distraksiyon osteogenezisi yöntemi ile tedavi sonuçları sunulmuştur. Olguların yaş ortalaması 24,8 (19-36 yaş) olup sadece l olgu bayan idi. Olgular ameliyattan sonra ilk ay haftalık, daha sonra aylık radyografik kontrollerle ortalama 40,7 ay (24-72 ay) takip edilmiştir, sonuç olarak eksternal fiksatör seçimi, cerrahın kişisel tercihine, bilgi ve tecrübesine bağlıdır. Olgularımızın çoğu yüksek enerjili ateşli silah yaralanması olup birçoğu da amputasyon gerektirebilecek, tedavisi zor kırıklardı. İlizarov Sirküler Eksternal Fiksatörü ve yöntemi ile bu kemik defektlerinin kapatılmasında oldukça etkili bir yön-tem olduğu sonucuna varılmıştır.
