Taban çöküklüğü ve topuk dikeninin topuk ağrısı etiolojisindeki rolü

Abstract

Çalışmada 29 tanesi iki taraflı olmak üzere topuk ağrısı yakınması olan, 20 ile 60 yaşları arasındaki 73 hastanın ayak yan radyografileri kalkaneal eğim açısı ve topuk dikeni yönünden incelendi ve 120 kişilik bir kontrol grubuyla kıyaslandı. Normal kontrol grubunda erkeklerde kalkaneal eğim açışı ortalaması 22.01 derece, kadınlarda ise 18.76 derece bulundu ve bu farkın istatistiksel olarak anlamlı olduğu saptandı (p<0.01). Topuk ağrısı yakınması olan hastalarda kalkaneal eğim açısı ortalaması 16.72 derece bulundu ve bunun normal olgulara kıyasla anlamlı oranda düşük olduğu belirlendi. Topuk ağrısı olan 73 hastanın 43 tanesinin (% 58) aşırı kilolu olduğu ve bu oranın normal bireylere oranla anlamlı ölçüde yüksek olduğu bulundu (%34) (p<0.00l). Normal kontrol grubunda topuk dikeni görülme sıklığı % 8.3, topuk ağrısı olan hastalarda ise %60.2 olarak saptandı (p<0.001). Kadınlarda, aşırı kilolu bireylerde ve yaş arttıkça kalkaneal eğim açısı azalarak, topuk dikeni oluşumunun arttığı ve bunun topuk ağrısı gelişimine neden olabileceği sonucuna varıldı.

The relationship between pes planus and calcaneal spur to plantar heel pain

Abstract

Hindfoot lateral radiographs of 73 patients with plantar heel pain, 29 of whom have bilateral involvement ( 102 feet ) ranging in age from 20 to 60 were studied and compared with 120 normal subjects ( 240 feet ). The mean normal calcaneal pitch in male was 22.01¡, female was 18.76¡. This difference was statistically significant (p<0.01). The mean calcaneal pitch in patients with plantar heel pain was 16.72¡, which was significantly lower than in normal subjects. While 43 out of 73 patients with heel pain (% 58) were overweight, the incidence for normal control group was (%34) (p<0.001). The incidence of calcaneal spur in normal subjects was 8.3 % and in patients with plantar heel pain 60.2% (p<0.001). We conclude that aging, excessive weight and female gender may be important factors effecting the lowering the pitch and the increasing of spur formation which may cause plantar heel pain.
