Biomechanical evaluation of the percutaneous pinning of unstable extra-articular fractures of the distal radius (A biomechanical study on an embalmed cadaver of the forearm)

Abstract

Extraarticular displaced distal radius fractures were created on the forearms of the 28 embalmed cadevers. Four kinds of configurations of stabilizations were performed by using 4,3,2, and 1 Kirschner wires.Stabilization tests were done by using the 600 kN Universal Testing Machine of the Servo-Hydraulic type named Dartec.A force was applied and displacement at the fracture sides measured.Results were evaluated statistically.The samples which were stabilized by one Kirschner wire were detected to be unstable.The ones which were stabilized by two crossed Kirschner wires were consistently stable.When three and four Kirschner wires were used it was noticed that more rigid stabilization was achieved;But the results were not significantly different from the samples which 2 Kirschner wires were used. In conclusion,when minimum of 2 Kirschner wires were used for the fixation of the displaced extraarticular distal fractures of the radius,an adequate and safe stabilizaton could be achieved.

Ekstraartiküler stabil olmayan distal radius kırıklarında perkütan çivilemenin biomekanik olarak değerlendirilmesi (Kadavra ön kolunda biomekanik çalışma)

Abstract

Ekstraartikular deplase distal radius kırığı oluşturulan 28 kadavra ön kolunda repozisyon yapılarak 4 çeşit konfigurasyonda stabilizasyon yapılmıştır. Bunlar sırasıyla 4, 3, 2 ve 1 tane Kirschner teli kullanılarak sağlanmıştır. Test Danec marka 600 kN servo-hidrolik tip üniversal test cihazı ile kuvvet uygulanarak yapılmış ve kırık bölgesindeki deplasman ölçülmüştür. Sonuçlar istatistiksel olarak değerlendirilmiş olup tek Kirschner ile stabilizasyon yapılan örneklerin tüm testlerde anstabil olduğu gozlenmiştir. Çapraz iki Kirschner ile tespit yapılan konfigurasyonlarda daha devamlılık gösteren bir stabilite dikkati çekmiştir. Üç ve 4 tane Kirschner ile stabilizasyon yapılan konfigürasyonların tümünde daha rijid bir stabilizasyonun mevcut olduğu görülmüştür. Fakat 2 tane Kirschner kullanılarak yapılan örnekler arasında çok aşırı bir fark bulunamamıştır. Bundan dolayı distal radiusun ekstraartikular deplase kırıklarında minumum 2 Kirschner ile tespitin yeterli ve güvenilir bir stabilizasyon sağladığı görülmüştür.
