Results and problems of interlocking nail stabilization in femoral shaft fractures

Abstract

In this study 22 patients with femoral shaft fractures due to primary or pseudoarthrosis were operated with interlocking intramedullary nail. These patients were followed for at least 18 months, have been observed for clinical and functional results with the complications they manifested. Patients mean age was 36.2 years . In 22 patients, Grosse-Kempf interlocking nail was applied. Fifteen of them had primary fractures and 7 had psudoarthrosis related with insufficient fixation. Ten of the applications were dynamic while other 12 were static. Seven of the static applications were dynamized later. Mean dynamization time was 19 weeks. Union was seen in 13 of 15 patients (86.7%) with primary fractures at a median of 23.1 weeks. The median union time was 21.8 weeks in all of the patients with pseudoarthrosis. Functional results were classified as excellent and good in 80% patients with primary fractures and 71.5% patients with pseudoarthrosis, according to the criteria given by Thoresen. Twenty fractures healed without problem whereas 5 manifested delayed unions. There were 2 nonunions. We have experienced distal screw irritation in 13 patients, thigh pain in 7 patients, shortening in 4 patients, infection in 3 patients and some other complications. We have found the interlocking intramedullary nail fixation technic to be useful in treating patients with high-energy fractures and pseudoarthrosis. The technic, however, would also be adversial to the patient by extending the operation time and to the operating team for the highly exposed x-ray doses. Shortening is an other particular complication following the dynamization and so is the screw site irritation. Those are why we should strongly suggest the very detailed preoperative planning and adequate equipment for interlocking nail stabilization indication.

Femur cisim kırıklarında kilitli intramedüller çivileme sonuçları ve koplikasyonları

Abstract

Bu çalışmada Mayıs 1994-Aralık 1996 tarihleri arasında primer veya implant yetmezliği sonucu gelişen femur kırıklarında kilitli intramedüller fiksasyon uygulanan ve ortalama 30 ay süreyle düzenli kontrolları yapılan 22 hastanın klinik ve fonksiyonel sonuçlarıyla, hastalarda gözlenen sorunlar incelendi. Yaş ortalaması 36.2 olan hastaların 15'ine primer femur kırığı, 7'sine psödoartroz tanısıyla Grosse-Kempf tipi kilitli intramedüller çivi ile fiksasyon uygulandı. Kırıkların 5'i 1/3 proksimal, 11'i 1/3 orta, 6'sı 1/3 distal femur yerleşimliydi. Hastalaın 10'una dinamik, 12'sine statik çivileme yapıldı. Statik çivileme yapılan hastaların %7'sinde çivi, ortalama 19 hafta sonra dinamize edildi. Primer kırıklı hastaların 13'ünde (%86.7) ortalama 23.1 haftada, psödoartrozlu hastaların ise tümünde (%100) ortalama 21.8 haftada kaynama sağlandı. Thoresen kriterlerine göre yapılan final değerlendirmesinde; primer kırıklı hastaların nonunion, 5 hastada kaynama gecikmesi tespit edildi. Ayrıca 13 hastada distal tespit vidası bölgesinde irritasyon, 7 hastada uyluk ağrısı, 4 hastada kısalık ve 3 hastada yüzeyel enfeksiyon başta olmak üzere 11 farklı komplikasyonla karşılaşıldı. Çalışmanın sonucunda parçalı ve instabil femur kırıkları ile psödoartroz vakalarında kilitli intramedüller çivi uygulamasının iyi sonuçlar verdiği saptandı. Ancak aynı tekniğin, uygun kırık masası ve C-arm düzeneğinin bulunmadığı zamanlarda hem ameliyat süresini uzattığı, hemde cerrahın aşırı X ışınına maruz kalmasına neden olduğu gözlendi. Saptanan bu komplikasyonlar nedeniyle kilitli intramedüller tespit endikasyonu konurken, dikkatli preoperatif planlama ve hazırlık ile deneyimli ekip ve yeterli ekipman ihtiyacının gözden uzak tutulmaması gerektiği sonucuna varıldı.
