The relationship between risk factors, clinical findings and ultrasonographic findings in developmental hip dysplasia

Abstract

In this study, the relationship between the ultrasonographic findings and risk factors, clinical findings in 150 babies (average age 3.9 months) whose both hips were ultrasonographically examined by the Graf method was analyzed. Among 300 examined hips 34 (11%) of them were considered to be ultrasonographically pathologic. Rate of occurrence of developmental hip dysplasia (DDH) in babies with positive and negative risk factors was found to be 28.1% and 8.1% respectively (p<0.01). In babies with dysplastic hips positive family history and breech presentation were the most common two risk factors. Rate of occurrence of DDH in hips with abnormal and normal clinical findings (except asymmetry of thigh folds) was found to be 46.7% and 2.5% respectively (p<0.001). Limitation of abduction was the most common clinical finding in pathologic hips. In the presence and absence of asymmetric thigh folds the rate of occurrence of DDH was 30.8% and 11.7% respectively (p<0.05). When risk factors and clinical findings were considered together, the rate of occurrence of DDH was found to be the highest (58.6%) in cases with positive risk factors and abnormal clinical findings (including asymmetry of thigh folds) (p<0.005). In cases with normal clinical findings and with or without risk factors the rate of occurrence of DDH was less than 4%. In conclusion, in babies with abnormal clinical findings the physician should be alert on the presence of DDH. It should also be remembered that the rate of occurrence of DDH is higher in babies who have risk factors than in ones who have not any. These babies at risk must be ultrasonographically screened to lessen the incidence of DDH.

Gelişimsel kalça displazisinde risk faktörleri ve klinik muayene bulguları ile ultrasonografik bulgular arasındaki ilişki

Abstract

Bu çalışmada her iki kalçaları Graft yöntemiyle ultrasonografik olarak muayene edilen ortalama yaşı 3.9 ay olan 150 bebekteki ultrasonografik bulgular ile risk faktörleri ve klinik muayene bulguları arasındaki ilişki araştırıldı. İnce-lenen 300 kalçadan 34 tanesi (%11) ultrasonografik olarak patolojik değerlendirildi. Gelişimsel kalça displazisi (GKD) için risk faktörü taşıyan ve taşımayan bebeklerde GKD görülme oranı sırasıyla %28.1 ve %8.1 olarak saptandı (p<0.01). Displazik kalçalı olgularda aile öyküsü ve makat geliş öyküsü en sık görülen iki risk faktörüydü. Klinik muayenesi (pili asimetrisi hariç) anormal ve normal olan kalçalarda GKD görülme oranı sırasıyla %46.7 ve %2.5 olarak bulundu (p<0.001). Patolojik kalfalarda abduksiyon kısıtlılığı en sık görülen bulguydu. Pili asimetrisi olan ve ol-mayan olgulardaki GKD görülme oranı sırasıyla %30.8 ve %11.7 idi (p<0.05). Risk faktörleri ve klinik muayene bul-guları beraber değerlendirildiginde GKD görülme oranı hem risk faktörü pozitif hem de anormal klinik bulgusu (pili asimetrisi dahil) olan olgularda en yüksek (%58.6) olarak bulundu (p<0.005). Klinik bulguları normal olan olgularda risk faktörü olsa da olmasa da GKD görülme oranı %4'ün altında bulundu. Sonuç olarak anormal klinik muayene bulgularının varlığında GKD açısından dikkatli olunmalıdır. Ayrıca GKD görülme oranının risk faktörü olan bebeklerde daha yüksek olduğu akıldan çıkarılmamalıdır, özellikle bu riskli bebeklerin ultrasonografik olarak mutlaka taranması GKD görülme sıklığının azaltılması bakımından gereklidir.
