Abstract
22 patients which were treated surgically after prewalking age between Jan 1985-Dec 1992 in our clinic were evaluated in this study. All patients recieved modified Salter osteotomy and femoral shortening at one stage without traction. Mean operation age was 2.7 years( R: 1.5-6 years) and mean follow-up was 6.5 years (R: 5years 2 months-8 years 6 months). X-rays of the hips which were taken 6 weeks after the operation were scored according to our grading system. In this system centralization of femoral head, placement of femoral osteotomy, displacement type of distal fragment of Salter osteotomy, the amount of increase in acetabular index and final degree of collodiaphyseal angle were graded. The relationship between the final score which was obtained by this grading system and prognosis during our late control of the hips were evaluated statistically. According to Barrett-Modified Mc Kay criterias 77% excellent, 14% good and 9% fair results were obtained. We classified hips of our cases as 82% excellent, 4.5% good, 4.5% fair and 9% poor results radiologically according to Severin classifications. We obtained a meaningfull relationship between our early grading system and Barrett-Modified Mc Kay and Severin Criterias. We conclude that it is possible to identify the late prognosis of our operated DDH cases after prewalking age by our early grading system.

Abstract
Ocak 1985-Aralık 7992 yılları arasında kliniğimizde gelişimsel kalça çıkığı sebebi ile daha önceden hiç tedavi görmemiş ve ilk tedavi olarak radikal redaksiyon uygulanan 22 kalça çalışmaya alınmıştır. Ortalama ameliyat yaşı 2.7 yıl (1.5-8 yıl) ve ortalama takip süresi 6.5 yıl (5 yıl 2 ay-8 yıl 6 ay) idi. Olgularımızın ameliyat sonrası 6. haftada çekilen radyografileri geliştirdiğimiz radyolojik unsurlara göre değerlendirilmiştir. Bu sistemde; santralizasyon, femoral osteotominin niteliği, kollodiafizer açının derecesi, Salter osteotomisinin niteliği ve asetabüler indekste düzelme miktarı gibi alt başlıklar bulunmakladır. Bu skorlama sistemi ile son kontroldeki prognoz arası ilişki değerlendirilmiştir. Barrett-Modifiye Mc Kay klinik değerlendirmesine göre; %77 mükemmel, %14 iyi ve %9 orta sonuç elde edilmiştir. Severin radyolojik sınıflamasına göre; %82 mükemmel, %4.5 iyi , %4.5 orta ve %9 kötü sonuç elde edilmiştir. İleri yaşlarda ameliyat edilen olgularda Tip III avasküler nekroz oranının arttığı istatistiksel olarak saptanmıştır. Barrett-Modifiye Mc Kav ve Severin değerlendirme kriterleri ile geliştirdiğimiz sınıflama arasında da istatistiksel olarak anlamlı ilişki tespit edilmiştir. Sınıflama sistemimiz ile ameliyat sonrası 6. haftada çekilen kalça radyografileri değerlendirilerek yürüme sonrası ameliyat edilen gelişimsel kalça çıkıklı olguların ileri dönem prognozları hakkında bilgi sahibi olunabilmektedir.
