Peroneal nerve lesions after high tibial osteotomy (Postoperative electrophysiological study)

Abstract

One of the alternatives for surgical treatment of gonarthrosis is high tibial osteotomy. Peroneal nerve lesions may occur as complications after these operations. The frequency, the level and the reasons of these complications were surveyed in this study. Between October 1995- December 1997, eleven patients who suffered medial gonarthrosis were applied high tibial valgization osteotomy by Maquet-barrel-vault technique. Electrophysiological studies (Electomyography+nerve conduction studies) were applied postoperatively in order to search whether nerve lesions occurred or not. On the second electrophysiological control study, of the three patients (%27,3) who had peroneal nerve lesions previously, one totally recovered, the other’s motor lesion was recovered, but sensorial action potential loss was stable. In this study, these nerve lesions which would be overlooked by mild weakness and hypoesthesia in early postoperative period, have been seen with a higher rate than known with electrophysiological study.

Yüksek tibial osteotomide peroneal sinir lezyonları (Postoperatif elektrofizyolojik çalışma)

Abstract

Gonartrozun cerrahi tedavi seçeneklerinden olan yüksek tibial osteotomi operasyonunun olası komplikasyonlarından periferik sinir lezyonlarının kliniğe yansıdığından daha sık olup olmadığı, düzeyi ve nedeni araştırıldı. Ekim 1995- Aralık 1997 tarihleri arasında medial gonartroz nedeniyle Maquet-barrel-vault tekniği ile yüksek tibial valgizasyon osteotomisi uygulanan 11 hastaya postoperatif olarak yapılan elektrofizyolojik (Elektromiyografi ve sinir ileti çalışmaları) incelemeler ile peroneal, tibial ve süral sinir lezyonun gelişip gelişmediği incelendi. Üç hastada (%27,3) erken dönemde saplanan peroneal sinir lezyonu ikinci kontrolü yapılan bir hastada tamamen geriledi, diğer bir hastaca motor lezyonu geriledi ancak duysal aksiyon potansiyeli kaybı kalıcı oldu. Erken dönemde hafif güç kaybı ve duyu azlığı gibi minör bulgularla spontan iyileşebilen ve gözden kaçabilen bu lezyonların, yapılan elektrofizyolojik tetkikler ise tahmin edilen değerlerden daha yüksek oranlarda olduğu görüldü.
