The application time for tourniquet in extremity surgery and metabolic effect of accompanying diseases

Abstract

30 patients between 31-65 years of age who were treated with elective arthroscopic knee surgery at Haydarpaşa Numune Hospital were included in the study. Thirteen of the patients were men and 17 were women. Arterial blood samples were collected for gas analysis just before the application of pneumatic tourniquet which was used at 300 mmHg pressure, immediately and 20 minutes after the release of the tourniquet. PCO2, HCO3, pH, BE(base excess), variations in K values were recorded. Results were evaluated with "student’s t-test" and "p values" of <0.05 was considered significant. When the cases under or over 1 hour were excluded in reference to tourniquet time, there was no significant difference in arterial blood gases except pH values. At the end of 20 minutes, other values were observed to reach initial values. Contrary to this, in the group with accompanying disease it was observed that there was a significant difference in the values (pCO2 BE, pH) of the 3rd period. Maximum tourniquet time was approximately 2 hours in our series. We didn’t see any difference in patients where tourniquet application was longer or shorter than 1 hour and we concluded that tourniquet application at 300 mmHg pressure until 2 hours is safe in healthy adults and that in patients with concomitant disease this time sould be limited with 1 hour.

Ekstremite cerrahisinde turnike uygulama zamanı ve eşlik eden hastalıkların metabolik etkisi

Abstract

Haydarpaşa Numune Hastanesi'nde elektif artroskopik diz cerrahisi uygulanan 31-65 yas arası 30 hasta çalışmaya alındı. Hastaların 13'ü erkek, 17'si kadındı. Hastalara 300 mmHg basınç ile pnömotik turnike uygulanmadan hemen önce, turnike açıldıktan hemen sonra ve 20 dakika sonra arteryel kan gazı örnekleri alındı. PC02, H-C03, pH, BE (baz açığı). K değerlerindeki değişiklikler kaydedildi. Sonuçlar student's-t test ile değerlendirildi. P<0.05 anlamlı kabul edildi. Vakalar 1 saatlik turnike süresini dikkate alarak değerlendirildiğinde bu sürenin altında ve üstünde turnike uygulanan vakalarda 3. dönem sonunda arter kan gazlarında pH değerleri dışında anlamlı bir farklılık tespit edilmedi. 20 dakikanın sonunda diğer değerlerin başlangıç değerlerine ulaştığı gözlendi. Buna karşılık yandaş patolojileri olan hastaların olduğu grupta 3. dönem (pC02, BE, pH) değerlerinde anlamlı farklılık olduğu gözlendi. Maksimal turnike süresinin yaklaşık 2 saati bulduğu vaka serimizde 1 saatin altında ve üstünde turnike uygulamasının anlamlı bir fark yaratmaması 2 saate kadar 300 mmHg basınçlı turnike uygulamasınıns sağlıklı erişkinlerde güvenle uygulanabileceği, yandaş hastalığı olanlarda bu süreyi 1 saat civarında sınırlamanın daha emin olacağı kanaatine vardık.
