Early mobilization versus cast in the treatment of lateral ankle sprains

Abstract

Lateral ankle sprains or inversion injuries are frequently seen in athletes and also in general population. They approximately account for 90% of all ankle sprains. Lateral ankle sprains are very common injuries, despite this no single method of treatment has been accepted. In grade 2 injuries cast is frequently applied for three weeks. We tried to find out if cast application is proper for treatment of G2 lateral ankle sprains. Forty-two patients with a lateral ankle sprain were randomly selected for one of two treatment groups. The early mobilization group received an elastic bandage and no weightbearing was allowed for 2 days. Patients in the immobilization group were placed in a cast for 3 weeks. Follow-up evaluations were performed at 3 weeks, 6 weeks, 9 weeks and average 6 months after the initial injury. Pain, swelling, echymosis, instability were reviewed. Patients in the early mobilisation group had less pain at 3 weeks. 2 patients (%9) in the immobilization group resprained their ankles. 12 days after the injury, all the patients in the early mobilization group were able to return full work. We conclude that in first time G2 ankle sprains, early mobilization allows earlier return to work and well tolerated for patients.

Lateral ayak bileği burkulmalarmın tedavisinde alçı ile erken hareketin karşılaştırılması

Abstract

Lateral ayak bileği burkulması veya inversiyon yaralanmaları sıklığı genel toplumda ve sporcular ara-sında fazladır ve tüm ayak bileği burkulmalarının %90'ını teşkil eder. Lateral ayak bileği burkulmaları oldukça sık görülmelerine rağmen tedavilerinde geçerli tek bir yöntem kabul edilmiş değildir. Grade 2 burkulmalarda 3 hafta süre ile alçı uygulaması sık olarak uygulanmaktadır. Bu çalışmada Grade 2 ayak bileği burkulmalarında alçı uygulamasının uygun olup olmadığı araştırıldı. Lateral ayak bileği burkulması geçirmiş 42 hasta rastgele erken mobilizasyon ve alçı grubu olarak, 2 tedavi grubuna ayrıldı. Erken hareket grubuna elastik bandaj uygulandı, 2 gün süre ile yük verilmedi. İmmobilizasyon grubundaki hastalara 3 hafta süre ile kısa bacak alçı uygulandı. Hastalar burkulmadan sonraki 3. 6. 9. haftalarda ve ortalama 6. ayda kontrole çağrıldı. Ağrı, sislik, ekimoz ve instabilite değerlendirilen. 3. haftadaki kontrollerde erken hareket grubunda ağrının daha az olduğu gözlendi, immobilizasyon grubunda 2 hastada tekrar burkulma saptandı. İşe dönme erken hareket grubunda daha çabuk oldu (ortalama 12. gün). Sonuç olarak lateral ayak bileği G2 burkulmalarında erken hareket tedavisi işe erken dönmeye izin vermesi ve ekonomik olması sebebi ile tercih edilmelidir.
