Use of peritalar release operation in neglected convex pes valgus (Early results of 7 feet)

Abstract

The aim of this study was to analyze the early results of 7 feet with neglected convex pes valgus treated by peritalar release. Seven convex feet of 5 children (4 female and 1 male) were surgically treated between the dates November 1992 and August 1997. Average age at operation was 4 years (1.5-7.5 years). None of the feet had a previous treatment. Two patients were involved bilaterally. In three patients associated conditions (diastematomyelia, sacral agenesis and myelomeningocel) were present. Surgical procedure consisted of reduction and fixation with K-wire of talonavicular joint after the release of all peritalar soft tissue contractures. While performing peritalar release. Cincinatti incision was used in 3 and combined posteromedial and lateral incisions in 4 feet. Two feet were reoperated. Avascular necrosis of talus, pes calcaneus and dorsal subluxation of navicula were the observed complications in 3 feet. Follow-up period ranged from 12 to 64 months with an average of 31 months. At the latest follow-up all feet were evaluated by Walker and associates' scoring system. According to this scoring system five of seven feet were considered to have a satisfactory outcome. All of these feet were operated on between 1.5 and 4 years of age. Two feet of one child had unsatisfactory results and age at operation was 7 and 7.5 years respectively. In conclusion, peritalar release was considered to be a satisfying surgical procedure in children between 1.5 and 4 years of age who had convex pes valgus.

Gecikmiş konveks pes valgusta peritalar gevşetme ameliyatının yeri (7 ayağın erken dönem sonuçları)

Abstract

Bu çalışmanın amacı peritalar gevşetme yöntemiyle tedavi edilen 7 gecikmiş konveks pes valguslu ayağın erken dönem sonuçlarını incelemekti. Kasım 1992 ile Ağustos 1997 tarihleri arasında 5 çocuğun (4 kız 1 er-kek) 7 konveks ayağı cerrahi olarak tedavi edildi. Ortalama ameliyat yaşı 4 yıldı (1.5-7.5 yıl). Hiçbir ayak daha önce tedavi görmemişti. 2 hastada tutulum iki taraflıydı. Üç hastada ek patolojiler (diastematomyelia, sakral agenezi ve myelomeningosel) vardı. Cerrahi girişim tüm peritalar yumuşak doku kontraktürlerinin gevşetilmesinden sonra talonaviküler eklemin K-teli ile fiksasyonunu içermekteydi. Peritalar gevşetme yapılırken 3 ayakta Cincinatti kesisi, 4 ayakta ise kombine posteromedial ve lateral kesiler kullanıldı. İki ayak tekrar ameliyat edildi. Talus avasküler nekrozu, pes kalkaneus ve navikulanın dorsale subluksasyonu 3 ayakta görülen komplikasyonlardı. İzlem süresi 12 ile 64 ay arasında değişmekte olup ortalama 31 aydı. Son izlemlerinde tüm ayaklar Walker ve arkadaşlarının puanlama sistemine göre değerlendirildiler. Bu sisteme göre 7 ayağın 5 tanesinde sonuç tatminkar olarak değerlendirildi. Tüm bu ayaklar 7.5 ile 4 yaş arasın-da ameliyat edilmışlerdi. Bir çocuğun her iki ayağında başarısız sonuç elde edildi ve bu ayaklar sırasıyla 7 ve 7.5 yaşında ameliyat edilmişlerdi. Sonuç olarak peritalar gevşetme 1.5-4 yaş arasındaki konveks pes valguslu çocuklarda başarılı bir cerrahi yöntem olarak kabul edildi,
