Evaluation of sensory recovery of thenar and cross finger flaps used in finger tip injuries

Abstract

Objectives: Sensation of cross finger flaps and thenar flaps were evaluated in patients who underwent surgery for fingertip injuries.
Methods: The study included 34 patients who had fingertip injuries due to occupational accidents.Sensation of the flaps were assessed using Semmes-Weinstein monofilament test, static and dynamic two-point discrimination tests, vibration test, and cold intolerance.
Results: Semmes-Weinstein monofilament test showed normal in 10 patients (29.4%), reduced light touch in 22 patients (64.7%), and reduced protective sensation in two patients (5.88%). With static two-point discrimination test, eight patients (23.5%) were evaluated as normal, 18 patients (52.9%) as mild, and eight (23.5%) patients as poor. Dynamic two-point discrimination test results were 5 mm or less in eight patients (23.5%), between 6 and 8 mm in 24 patients (70.5%), and 10 mm in two patients. According to the correlation of the sensibility tests, S2 was obtained in two patients, S3+ in 20 patients and S4 in 12 patients. No significant correlation was found with regard to sensory recovery between thenar and cross finger flaps.
Conclusion: We concluded that cross finger and thenar flaps are still the procedures of choice for fingertip injuries due to their ease of performance obviating the need for microsurgical team or equipment, and to satisfactory healing with almost normal sensory reinnervation.

Parmak ucu yaralanmalarında kullanılan çapraz parmak flebi ve avuç içi fleplerinde duyusal iyileşmenin değerlendirilmesi

Abstract

Amaç: Parmak ucu yaralanması ile başvuran ve çapraz parmak flebi veya avuç içi flebi uygulanan hastalarda flebin duyu yönünden gelişimi değerlendirildi.
Çalışma planı: İş kazası nedeniyle başvuran ve parmak ucu yaralanması nedeniyle çapraz parmak flebi veya avuç içi flebi uygulanan 34 hastada Semmes-Weinstein monoflaman testi, statik ve hareketli iki nokta ayırım testleri, vibrasyon testi uygulandı ve soğuk intoleransına bakıldı.
Sonuçlar: Semmes-Weinstein monoflaman testine göre 34 hastanın 10’unda (%29.4) normal, 22’sinde (%64.7) azalmış hafif dokunma, ikisinde (%5.88) azalmış koruyucu duyu sonucu alındı. Statik iki nokta ayırım testine göre, sekiz hastada (%23.5) normal, 18 hastada (%52.9) orta, sekiz hastada (%23.5) kötü sonucu alındı. Dinamik iki nokta ayırım testi, sekiz hastada (%23.5) 5 mm ve altında, 24 hastada (%70.5) 6-8 mm arasında, iki hastada 10 mm bulundu. Duyu testlerinin sonuçlarının korelasyonunu gösteren sınıflamaya göre, iki hastada S2, 20 hastada S3+ ve 12 hastada S4 sonucu elde edildi. Çapraz parmak flebi ve avuç içi flepleri arasında duyu iyileşmesi bakımından istatistiksel olarak anlamlı bir fark bulunamadı.
Çıkarımlar: Çapraz parmak flebi ve avuç içi flepleri, kolay uygulanır olmaları, mikrocerrahi teknik ve ekipman gerektirmemeleri ve normale yakın bir duyusal iyileşme sağlamaları nedeniyle parmak ucu yaralanmalarında tercih edilebilir yöntemlerdir.
