Principles of surgical treatment in patients with complicated syndactyly

Abstract

Methods: Thirteen patients (10 females, 77%; 3 males, 23%) underwent surgery for congenital complicated syndactyly. Mean age of operation was 6.3 years (range 0.5-21 years). Four patients had bilateral syndactyly. According to the anatomo-pathologic classification of syndactyly, patients were classified as simple-incomplete-complicated (n=8), simple-complete-complicated (n=2), and complex-complete-complicated (n=3). A total of 35 operations was performed in the patient group in order to reconstruct web spaces and correct associated anomalies. Web spaces were reconstructed using dorsal rectangular flaps.
Results: Postoperatively, development of web creeps was noted in only two patients.
Conclusion: Patients with complicated syndactyly must be operated early in childhood and full-thickness grafts should be the preferred material to be used in operation.

Komplike sindaktili olgularında uyguladığımız cerrahi tedavi prensipleri

Abstract

Amaç: Konjenital komplike sindaktili tanısıyla cerrahi tedavi uygulanan hastalar değerlendirildi.
Çalışma planı: Konjenital komplike sindaktili nedeniyle 13 hastaya (10 kız, %77; 3 erkek, %23) cerrahi tedavi uygulandı. Hastaların ortalama ameliyat yaşı 6.3 (0.5-21 yaş) idi. Dört hastada (%28.6) iki taraflı tutulum vardı. Sindaktilinin anatomo-patolojik değerlendirilmesine göre bu hastaların sekizi basit-inkomplet-komplike, ikisi basit-komplet-komplike, üçü kompleks-komplet-komplike olarak sınıflandırıldı. Gerek web’lerin açılması gerekse diğer el anomalilerinin düzeltilmesi amacıyla 13 hastaya 35 seans ameliyat uygulandı. Web’lerin oluşturulmasında dorsal rektangüler flep kullanıldı.
Sonuçlar: Yalnızca iki hastada yeniden web oluşmasına rastlandı.
Çıkarımlar: Komplike sindaktilisi olan hastalar mümkün olduğunca erken yaşta ameliyat edilmeli ve ameliyatta tam kalınlıkta deri grefti kullanılmalıdır.
