Reconstructive treatment modalities for congenital femoral deficiencies

Abstract

Congenital femoral deficiencies have a wide spectrum of occurrences ranging from simple hypoplasia to complete femoral aplasia. They are often associated with congenital shortening of the tibia and fibular deficiency. There is no current consensus on the classification and treatment of this entity. Existing classifications are neither prognostic nor helpful in treatment-planning. There are two diverse treatment modalities in the treatment of congenital femoral deficiencies, being prosthetic replacement surgery (Syme amputation or van Nes rotation-plasty followed by prosthetic fitting) and lengthening reconstruction surgery. In this study, biological reconstruction methods in the treatment of congenital femoral deficiencies are reviewed. The results of lengthening reconstruction surgery seem to be superior and more economical than those of prosthetic replacements.

Konjenital femoral yetersizliklerde rekonstrüktif tedavi alternatifleri

Abstract

Konjenital femoral yetersizliklerin spektrumu basit hipoplaziden tam femur aplazisine kadar sıralanır. Genellikle tibia kısalığı ve fibuler displazi birlikte görülür. Bu oluşumun sınıflaması ve tedavisiyle ilgili bir konsensus yoktur. İleri sürülen sınıflamalar prognostik açıdan ve tedavi planlaması yönünden yeterince yardımcı değildir. Konjenital femoral yetersizliklerin tedavisi için iki farklı tedavi yöntemi vardır. Prostetik replasman cerrahisi (Syme amputasyonu veya van Nes rotasyon-plastisini takiben prostetik donanım) ve uzatma rekonstrüksiyon cerrahisidir. Bu yazıda konjenital femoral yetersizliklerde uygulanabilecek biyolojik rekonstrüksiyon yöntemleri ele alındı. Uzun süreli takipler göz önüne alındığında, biyolojik rekonstrüksiyon sonuçlarının prostetik replasmana göre daha üstün ve ekonomik olduğu kanısına varıldı.
